Filed as kern/207432. As noted there, it looks like ctrlr->ioq is null
when nvme_ctrlr_intx_handler
starts looking there. I get a feeling people don't usually use INTx for
this driver though...

Is have an inkling that r293673 may be at fault here, can you try 
reverting that change and see if it fixes the issue?
