We've known security questions like what's your mother's maiden name were junk for over a decade, and yet Apple still adopted them after their breach in 2012. (Its public information; and how is Mom supposed to change her maiden name after a breach?). And Apple rejects location limited channels like walking into an Apple store with a picture ID, a credit card used with the account, and past bills.
