This change necessitates a change to section 4 of the template:

needs to be amended to mention iclas.txt.

And while we’re on the subject. When I filled out the form on Friday (the first 
time I’ve ever used it) I was confused by section 5:

The path "/repos/asf/incubator/donations” does not seem to exist in subversion.
