I'm using Junction Networks and Les.NET.

Junction Networks has local DID's (or their partner does anyway) and  
multiple inbound channels. Transferring was smooth.  No problems at  
all with them.

Les.NET has better rates and is slightly less reliable.  They seem to  
be scaling up lately.  Run by a guy in Canada, though the servers are  
in VA.  None of that seems to matter.  I use him for outbound, and  
fail over to Junction if needed.  Both support IAX and SIP.

I think all DID's have a small monthly fee, but most of my cost is  
per-minute charges.  I believe each might have a flat-rate program,  
but I'm a believer in metered usage.

I've heard that Broadvoice used to be great and got bought and is now  
abusive towards their customers.  Google knows more than I do.

I've been pretty happy with www.voipjet.com (for outgoing long distance
and international) and www.teliax.com (for remote phone numbers - call
origination, and long distance within NH - call termination). They both
support SIP and IAX - I use IAX given my nested firewall configuration.

Teliax has both "pay as you go" plans and monthly plans. voipjet has
just "pay as you go".

I'm looking into some others that provide international DIDs and have
sub-cent rates. But those all require me to sign NDAs.

I'm using Broadvoice as well, no complaints, but I don't really use it
all that much, also BYOD with a Sipura SPA-2000.

I've had VoicePulse in the past, and wasn't too impressed with them. The
call quality was fine, but has some issues with their billing system and
their web interface didn't feel very polished.

I used Broadvoice for about a year. They were great. We used their
BYOD (Bring Your Own Device) service and you can use an asterisk box
for that (I used a small little box HT-486).

It's good to use to try out since it was only $11/month after taxes
for about 60minutes outgoing and unlimited incoming calls a month.

We dropped them because we just never used it. After 3 rings it would
ring my cell and my wife and I just always used our cellphones.

http://www.wengo.com/) The client is open source, works on windows,
Apple and Linux.  Has video conferencing as well as audio conferencing.
Its messaging client covers all major protocols.  And its long-distance
POTS/VoIP is one of the least expensive and greatest coverage.

A guy in Manchester sells $13/mo service via Asterisk, it's what I use for
my own phone.  He only provides service to NH residents and doesn't
advertise.  I can forward you to him if you're interested.

I should also note that I'd prefer the service use standard protocols
(IAX, SIP, or similar) and that the service needs to have an open
source client.

and receive telephone calls over the Internet).

I just don't talk that much. :) If I had a choice of paying a monthly
