by Jacob Freedman. It was published by the Jacob Freedman Liturgy
Research Foundation, Springfield, Massachusetts, 1974.

A friend of mine bought a used copy a couple of years ago from one of
the online used booksellers, I think.

It has been officially OP for some years, but I know stores do seem to 
obtain a few every year.  Check with Cody's here in Berkeley, and your 
local Judaica stores.
