When we saw this happening it was slide related, especially with it happening 
across all platforms.  We use Cardinal Healthcare SuperFrost Plus and haven't 
had an issue.  Every time we have tried another brand we have seen this happen 
to some degree.

We have been experiencing problems with ghosting (inconsistent and/or no 
staining) mainly affecting our control area which is located at the bottom of 
the slide.  Random antibodies markers are being affected. We were using Fisher 
Scientific then TruBond slides. We are using Ventana Ultras X4 and XT in which 
it is occurring on all machines. Ventana has come in to troubleshoot, in which 
we have not reached a solution.

Any suggestions would be greatly appreciated

I am glad to see someone else is concerned about the upgrade.  The chance of an 
entire run aborting is a bit frightening, mainly for the loss of irreplaceable 
small biopsy tissue.  The possibility of an incorrectly stained slide is 
equally as frightening.  However, with appropriate controls an incorrect 
antibody should be detectable.  I don't believe that either option is good, but 
the only ones to work with.  The overlay on the other hand is something that we 
have never used.  When we first started with Vantage (we were early adopters) 
the overlay was not available.  When it was we tried it for a while and the 
only difference was that it gummed up the coverslipper if slides were in xylene 
too long before they were coverslipped.  We are still contemplating the upgrade 
as the resolution is tentative for early summer.

Job Summary:
The Histology Technical Specialist coordinates and monitors the day-to-day 
workflow of the histology area of pathology, serves as the first line resource 
for daily operations and assists the Assistant Director of Laboratory for 
Pathology with schedule management activities and provides technical - level 
services in the area of expertise. Histotechnologist duties include practicing 
clinical laboratory science in the specialty of histology to include 
immunohistochemistry.  May be required to perform other duties within the 
pathology department as directed by the pathologist or assistant director of 
Pathology.  May include clerical duties or duties within the cytology area of 
the laboratory depending on the availability of personnel.  The Histology 
Technical Specialist works with the Histology Technical supervisor and AD to 
establish Histology department processes, and reviews, updates and writes 
histology laboratory procedures to ensure the high quality of specimen 
preparations.  Respo
 nsible for recognizing issues or problems that require referral to the 
Pathologist.  Maintains performance improvement activities within the 
department and participates in quality improvement activities. Coordinates and 
monitors daily activities of histology workflow; assists AD in activities that 
may include interviewing, work allocation, orientation, training and competency 
assessment, contributing to performance evaluations and motivating employees to 
achieve peak performance. Demonstrates pro-active problem solving skills to 
include identifying a problem and taking appropriate action; knows when to seek 
guidance to complete troubleshooting protocol; communicates necessary 
information to AD.  Acts as a resource to customers, both within the department 
and outside the department. Accepts responsibility for own development to 
include cross-training in other functions or positions to ensure satisfactory 
operation of the department or to permit flexible staffing assignments.
Regulatory Requirements:

  1.  High School graduate or equivalent.
  2.  Graduated from accredited school of histotechnology or medical laboratory 
technology.
  3.  Current MLT, MT, HT, or HTL certification, i.e., American Society of 
Clinical Pathologists.
Skills:

  1.  Demonstrates the ability to coordinate multiple tasks with precision and 
flexibility to provide accurate results within the minimum turnaround time 
established by this laboratory.
  2.  Knowledge and skills related to laboratory computer information systems 
and word processing is desirable.
  3.  Must be able to work within a framework of a team concept to accomplish 
identified mission and goals.
  4.  Excellent verbal, written, and clerical skills

  1.  Ability to carry objects weighing up to 25 pounds.

We have 2 Microm HM355S motorized microtomes. We use the motor 
to move the block close to the blade, but turn the wheel manually when 
sectioning. Both are "jumping" forward on their own occasionally while 
sectioning, which is unsettling and chunks into the block. One of them also 
backs up on its own as you're manually turning the handle, just as you're about 
to get a ribbon.
The stepper motor has been replaced on one with no improvement. 
Any suggestions, remedies, or tales of similar issues and how they were 
resolved would be most appreciated.

[Histonet] Assigning cases to pathologists in EPIC

For anyone currently using EPIC Beaker or any other system where you assign 
pathologists at accession, please share how you assign cases.  We are still 
currently using transcriptionists who assign the case to a pathologist as they 
listen to them dictate.  For purposes of outstanding lists we would like to 
assign cases as soon in the process as possible.  Please share how you doings 
this.

We ran into this situation with TJC.  We had a very limited check off sheet.  
In effort to standardize processes across the laboratory we adopted a program 
very similar to the NY facility whose program is referenced in the TJC leading 
practice library.  It is very rigorous and time consuming but serves its 
purpose well.  Also, CAP has a competency assessment program specifically for 
histology that is a subscription based purchase regardless if you are 
accredited by them.

Our last discussion with TJC to clarify their definition of "regular interval" 
was to follow the manufacturers guidelines.  I recommend determining what 
manufacturer suggests with the use of the UV decon and write that in your 
policy that addresses this standard.  If you are following their guidelines, 
you should be in compliance.

We use super frost plus and have tried many different slides over the last 15 
years.  We always go back to SuperFrost plus.  Cardinal Healthcare carries them 
under their brand but they are made by Erie Scientific.  It is my understanding 
that there are very few companies that make slides, they are just branded 
differently.  Check the manufacturer and if its Erie Scientific and they are 
SuperFrost then they should work as well.

replaced them with another type..Just wondering what others think
