[Histonet] Formalin Fixation issues

2012-07-09 Thread Jill Cox
Hi Histonetters,
Is anyone having problems with breast fixation prior to processing in formalin?
For a couple of months now our breast specimens aren't fixing very well before
gross. Our Pathologist thinks they have changed something in the formalin
itself. We utilize 15/1 ratio and in some cases let fix over the weekend. This
has only been happening over the last couple of months and can't seem to figure
this out. Any advise or similar problems, would love to hear from you..

Re: [Histonet] Re: Formalin Fixation issues

2012-07-09 Thread Jill Cox
Yes Bob, the sections are good and thin. We use Scientific Products, 10% NBF.
Just seems like the Formalin is watered down. Going to try a different brand. I
have worked for Pathologist's that cram cassettes but this is not the case
here.

At this point I'm not sure I understand what Jill Cox's problem is.
Forgive me if I keep blaming the pathologist - since I am one! - but
in my experience most of these breast problems originate at the gross
desk.

Is your pathologist cutting the fatty breast tissue thin enough? This
is always a challenge, even after you've done it as long as I have. If
your pathologist is cramming the cassettes full of fat, then that's
where the problem lies.

Do you prepare your own neutral buffered formalin, or buy it? I
prepared a lot of it years ago, and supervised others who did, and I
learned the hard way that mistakes are easy to make when you brew your
own. If you buy your NBF ready made, as most people do nowadays, then
read the label carefully. Have you changed brands recently?

Breast specimens shouldn't be expected to fix before they're grossed,
or at least before they've been cut into thin slices. Delaying
fixation compromises immunostaining, to say nothing of H E. They
should be grossed as promptly as possible after they're received, and
should never sit over the weekend without dissection.

I seriously doubt that anything in the formalin has changed. It's your
technique that needs to change. Unfortunately, this is a difficult
 task to delegate.

Re: [Histonet] Jill's MSDS question: a tech's perspective

color. I ended up ordering a MSDS binder from LSS yesterday. I just wanted all

Subject: [Histonet] Jill's MSDS question: a tech's perspective

Hi Jill-- Congrats on the job!

The MSDS in paper vs. eCopy copy will come down to the opinion of your
inspector.  Like Wil says, he's passed before...and I've passed before but only
because we had both eCopies and hard copies.  Most of the larger for-profit and
major health system administrations require both by SOP regardless, for
liability reasons usually but please read on!

Better yet, think of the practical uses and real-world applications
of this rather than the 'do it because I have to' ones:

If you or one of yours gets splashed with something--how do you want the next
response to go?  If you only have eCopies and the power is down, or you are
having computer issues, or the PC is slow, or the printer on the network isn't
responding, then what? If it's your eyes or your skin that's about to be
irreversably burnt, wouldn't you want that information available in a TOTALLY
reliable manner and a copy available to grab, follow AND send to the emergency
room with you--RIGHT NOW?  Then add to it the liability for your company should
this be the case...and we all know when you most need your PC to work is when
it doesn't.

If I'm having trouble making a decision often exaggerating the potential
circumstances makes the answer far easier to come to.

If I am working in a lab I want hard copies alphabetized by both the mfc.
chemical name and the common name we use for it in the lab with an index with
the binders (Print the covers black on yellow paper and insert these in clear
view binders) on the benches in the space where we work.  Not on a computer or
off in the transcription area.  It may not be required by your inspector or by
your facility, but is that really why we have these in our labs in the first
place--because we're required to do so?

My four cents

2008-10-20 Thread Jill Cox
Hi Ann,
I also just purchased a Leica XL Autostainer and Leica CV 5000 coverslipper
recently from Medical Equipment Source after calling all of the usual companies
offering refurbished equipment.  The pricing is a lot less than everyone else
with wonderful customer service, fast shipping and  better warranty. Would
highly recommend!! I have set up 5 labs now and have used them all, this one is
definitely the winner!! Wish I would have known about them sooner!! And NO, I
am not getting any kickbacks, LOL...

Hi Ann,

Our lab purchased a Leica 2030 microtome (using disposable blades)through
Medical Equipment Source. I'd looked at several companies that sell
refurbished microtomes, and went with this company because of the cost
($3000) that included a warranty and a newer knife holder than the original
one (at my request). They were also very pleasant to deal with. I talked
to Luigi Mascio and he was good at keeping in touch by phone and email
while the unit was being refurbished for us.

We purchased the microtome back in May of this year (5 months ago) and have
had no problems with it thus far - it works beautifully.
