Is anyone having problems with moisture on the slide, bubbles and misaligned 
coverslips or multiple coverslips. If so, what are they being told from 
Ventana? What have they done to resolve issues?

I spoke to Joan at CAP just yesterday regarding this question.  I too, am 
getting ready for CAP, so I feel your pain!
She said you can use the benchmarks in the notes under the question in the 
checklist or you can used other published articles that offer benchmark data.  
Just state what your reference is and where it is from.  She also said you can 
use proficiency testing data.

We are having our inspection this spring and I am working to get all our 
procedures, etc. ready.  I am having trouble finding benchmark information for 
comparison for HER2 to comply with this question - ...the laboratory at least 
annually compares its patient results with published benchmarks,   We are 
using the Dako Herceptest.  I spoke with Dako tech services and they did not 
have any information.   What are other labs using for a benchmark.

As compared to a lawsuit, yes it's cheaper.  But, when trying to explain
to a budget committee that you need something to label things that you
can do by hand...they usually don't see the point.  This brings me back
to my original point of mislabelling things 2 times in a year...

 I'm somewhat surprised that many labs are still handwriting blocks and 
slides. If you are using a LIS, can it integrate printing blocks and 
slides? Is the cost too high to add the printing capability? The cost of

equipment is so cheap compared with one lawsuit.

It would also reduce the stress of loosing your job over a labeling 
mistake. Just seems like a win win for everyone.

[Histonet] Sakura MicroArray Apparatus

We are making our own research / immunohistochemistry  controls and have found 
that the tips, as well as the molds are becoming quite expensive. The tips ( 
especially in the hands of a resident) have a tendency to be rather soft and 
loose their shape rather quickly. Any other users having this issue? 

Anyone know of someone that provides this service for a fee? We would provide 
tissues, pay for block making.

If it weren't for a med-tech-turned-pathologist that took me up on my
interest in becoming trained in histology (1967), I would not have just
marked my 41st year as a histologist.  Back then, you just had to have
OJT and study independently for your written and practical tests.  I had
no idea what I was getting into but it was one of the absolute best
choices I ever made.  To pretend to quote some baseball player,
Histology been berry, berry good to me!

I have been charging for legal cases for years; other institutions there is no 
charge---patient carte issue. We have a $60.00 search fee ( transcription  
histology expense offset) ; if we recut- $25.00 for each HE; special stains- 
$50; immunohistochemistry- $75.00.

We do not charge for recuts or send outs. Our Medical Records department 
charges $1 per page for reports. Charging is something that we have discussed 
for many years, though. I am also interested in what other institutions doe. It 
takes a lot of time for packing and record keeping even if sending originals 
(for legal cases), not to mention the time to recut and stain and the cost to 
send them.

Any institutions / laboratories where bone marrows are performed have an issue 
with Bone Marrows and what department gets what revenue / workload, etc.

There are no CPT codes for the technical work that is performed by the 
hematology technicians only the Surgical Pathology charges.  (save for 85097- 
Bone marrow smear interpretation , a professional charge). Technically 
speaking, about 1/3 of the technical work is done in hematology and 2/3 in 
histology but we have a discussion as who is to get the revenue because only 
one charge master code can be attached to the test.

Frankly, the revenue  stays in the laboratory, but when work stats are 
performed and cost / test is being evaluated, the administrators want a true 
cost per test. If there are hundreds of bone marrows, then this revenue can 
become an issue between hematology and surgical pathology.

Funny, I thought I had this issue taken care of but we are once again 
discussing it! 

What are others doing to verify formalin fixation times? Is it good enough to 
have a blanket statement in the report that we are in compliance of the 6-48 
hour required time ( we have fixation time on container dictated into report  
we process on the weekend )  or is the actual hours of fixation ( time in 
formalin container+ time in processor = total fixation time) documented into 
the report?

And do you document ALL? So that even those that are unsuspected (reductions) 
get a fixation ( just in case).

[Histonet] Ventana Symphony

Would like some feedback on any users out there. Any change in lab is difficult 
with some employees. That said, I think that it will actually be a plus in the 
lab---when the process changes are accepted. Any users out there have any 
comments? We are in evaluation period  and have six more weeks remaining.
