Good Morning Beth,
The HE600 is a Roche product.  The slides are stained flat with reagents being 
applied to each slide individually instead of batch staining like other 
instruments.  This allows you to get the same staining quality on EACH slide.   
We have used the instrument in our lab since 2005 and got a second unit when we 
moved into a larger location 2 years ago.  Coverslips are removed by apply heat 
from a hot plate and we have no issues with the hematoxylin if we remove a 
coverslip.  We rarely have issues with coverslips that we need to remove one.

I would appreciate any feedback from anyone with experience using the Leica H&E 
600. I recently attended a demo of the unit and we are still hung up on the 
large slide tray it uses instead of the traditional slide racks.   I also spoke 
with another user who indicated that if you need to re-coverslip a slide it is 
very difficult to remove the existing coverslip and when you do the hematoxylin 
is removed from the stain ???  We already know we will need two units to handle 
our workload of approximately 900 H&E.  Any information would be greatly 
appreciated.

Good Morning,
I have recently developed a problem with contamination of some kind on my IHC 
slides.
The contamination is black clumps and lays on top of the tissue.  I have
been told it is bacteria but the Pathologist and I kind of doubt that.   I
cannot get a good picture of it to show.  It may not be on all the slides on 
the same run or even the same antibody slide.

I have decontaminated the whole system.  I have put fresh reagents on the
instrument.   This stuff looks like it would wash off at the end of the run
since it is sitting right on top of the tissue.  I do not see this stuff
elsewhere on the slide, only the tissue.   Does not matter if the tissue
was cut fresh or not.

I have tried everything I can think of to get rid of it and still have this 
issue.  I use DiH20 from the hospital system.  Not sure if this may be the 
problem.  I can have Engineering test the DiH20.

Any help would be greatly appreciated.

Chris,
We have been tracking blocks since we went live with Vantage over 10 years ago. 
 Roche (Ventana then) built a custom program for us to use at that time because 
they had not yet implemented it into Vantage.  In the past 2 years Roche has 
updated their software to allow documenting this information.  There are 
several options of how the scanning is performed.  For our lab we scan before 
we place the block into the formalin holding tanks, right after the 
PA/Resident/Grossing Tech have placed the tissue into the block.  I prefer it 
done there because I do not want my technicians exposed an extra time to 
formalin if the scanning is done right before the baskets are placed on the 
processors.  We have a manual log that is used to document the time the 
cassette are placed in and taken out of the processors.  We also track our 
blocks and slides into storage.  I hope this helps.

Hi Histonetters,

Just wondering if or how others are tracking/scanning racks of blocks onto 
processors. In this day and age of traceability with barcodes, we scan each 
block in each rack before loading onto a processor and recording which 
processor that rack went on to. Has anyone got a more efficient way of doing 
this? Assigning at grossing/cut-up is an option, but is an extra step for the 
PA/registrar and relies on knowing which processor is in use that day.

Looking in the crystal ball, will next gen processors automatically scan blocks 
as the first step? Load blocks, start the run and it produces a report of the 
blocks on that run? Interested to hear peoples thoughts.

Any recommendations on a protocol for testing for reproducibility on our 
microwave and documenting it for CAP?  We are a new lab and just setting up

yesterday Freida Carson, PhD became one of our Histology Angels.  I had the 
privilege of having her as my Histology Educator long before she wrote her 
first edition of Histotechnology.  But more precious to me than having her as 
my educator was that she was a friend.  She will be missed.  Thank you Freida 
for all that you have given to the Histology Profession and to me.  Rest In

[Histonet] Dr. Freida Carson Histology Lab

Good Morning Everyone,
On December 15, 2021 Baylor Scott&White Health Baylor University Medical Center 
will be naming the Anatomic Pathology Department after Dr. Freida Carson. She 
was a founding member of NSH and served as President and Speaker of the HOD for 
many years. At Baylor she was the department leadership for 35 years, opening 
one of the first NAACLS approved schools in the nation in 1971. Numerous 
Histotechnician have had the privilege to have been trained by her or have used 
one of the 5 editions of her Histology training book throughout the years. 
Naming the lab after her is a little thing compared to what she has done for 
our profession. I am excited that the Baylor Board of Trustees is honoring her 
with this prestigious tribute.

[Histonet] Beaker Tracking vs Vantage

are moving from SoftPathDX to Beaker.  Currently we have Vantage totally 
bi-directional with SoftPathDX for our Tracking/Patient Safety system.  Would

someone that had Vantage and went to Beaker can you give detail about the 
differences between the 2 and your preference.  Any helpful hints for a Beaker 
Build.

Good morning all,

I am hoping you can help me.  We are looking for a "freezing chamber/bath" for 
freezing blocks for Frozen Sections.  I have looked and can not find what I am 
looking for. What is everyone else using?  We currently are using a 
"Thermoflask" with 2 Methlybutane and dry ice... this is not working so 
well...the chamber itself is too narrow.

Any help would be greatly appreciated!!

Hi,
If tissue is fixed in formalin ( I do not know if it is 10% NBF) then put in 80 
% isopropanol  can the tissue be processed as if it was in ETOH? What are the 
effects to the tissue stored in isopropanol?

My interpretation of this is that for AP the "test" is the reporting of the 
results.  What is done in the Histology lab to prepare the slides is the 
pre-analytical portion of the test.  

For the revised ANP. 21395 the pathologist is mentioning in the report the 
results of the controls therefore it is in concurrent with the report.  We 
still check our results before taking the slides to the pathologist.

Just to shed some light on the subject on the matter to have things in 
perspective,  when I called CAP and here is what the Rep shared with me,  it 
wasn't the interpretation  or the result but rather the QA/QC of the result.  
See below:  now here is the rub on this, as a person doing Special Stains, IHC, 
Digital Path and ISH,,  we as TECH's QA/QC before we hand into the Pathologist, 
our controls are supposed to be reviewed by the tech to make sure the reaction 
took  place, we don't say it looks brown, or red, or hey the colorful,, WE 
confirm the reaction, cause if not we redo.  This is what ALSO the CAP states 
that we do because it does say Pathologist or Designee , and it's the QA/QC 
step.  By no means are we resulting or giving Diagnosis/interpretation.  This 
is where I am having trouble with this statement, about 
testing/interpretation,, it has nothing to do with that at all.  CLIA also 
doesnt define QA/QC , just resulting/interpretation.

They are the same Steps that are being followed on digital path, were imaging 
of the slides require a person of High Complexity testing to be doing the 
scanning.  Again this is another QA/QC step, so I get all the stuff about CLIA 
and reporting, but CAP is specific and we are doing the QA/QC of this testing.

I also had a conversation about whether Licensure was acceptable or not,,  
without meeting the requirements, and I was told NO,, they need to meet the 
minimum requirements as stated in CLIA for high complexity testing,, again 
something to look at.

I then called ASCP to ask them about where they stood with this and was told we 
only do the licensure not the regs.

I called CMS to get a better understanding stating the issue, as for them the 
person interpreting and reporting is the Pathologist, but if CAP requires that 
the QA/QC be done with those regs,, then that is more than is required, but 
that is their regulation and to be adhered too.

Funny thing then when you go the CLIA website you get the high complexity 
testing, you can see where the testing methodologies are and can see when the 
test system came about. 

I know this is going to be a debatable situation and I have to agree with Tim,, 
please standardize and let us know,, but it is clear what they are saying about 
results, testing , interpretation and now QA/QC..  

Thoughts anyone,, but I see that this is going to change histology as a whole, 
since now this is being added too the mix.  I am not saying it is right or 
wrong,, but it does put forth effort that we need to classify what we do and 
also try to align the test system accordingly, because QA/QC is almost 
everything,, you can put this on instrumentation, protocols, procedures, etc.  

Also what do we do with those that have been doing this for years,, that have 
the knowledge and also the background,, again I don't wish to open up PANDORA's 
box,, I also know we are all going to look at this differently,, but the facts 
are we are changing and if we do not own the processes we currently do, someone 
else will that's for sure.

histochemical stains, and studies using immunologic and ISH methodology, 
positive and negative controls are verified and recorded as acceptable prior to 
or concurrent with the reporting of patient results and records maintained.

NOTE: Controls must be verified and recorded as acceptable by a pathologist or 
designee (provided the designee meets high complexity testing qualifications).

Positive tissue controls must contain the component specific to the special 
stain that is being applied to the specimen.
Immunohistochemical tests using polymer-based detection systems (biotin-free) 
are sufficiently free of background reactivity to obviate the need for a 
negative reagent control and such controls may be omitted at the discretion of 
the laboratory director following appropriate validation.
