If a SLN is sent for frozen section (boo hiss) we treat as any other frozen and 
we typically cut two sections. If the pathologist is able to determine the 
lymph node is positive, we will only cut a permanent H&E once processed; we do 
not need to cut all the SLN levels. If the tissue appears negative on frozen 
then we will treat it as a routine SLN; we then cut the permanent H&E and an 
unstain, trim in about 100 um, cut an H&E and an unstain, trim in about 100 um 
and cut the final H&E. Hope this makes sense!

Hi Terri!

At the hospital I'm currently at, we do the same as Lacy's lab for permanent 
and frozen sections.  We also wait for the pathologists to put in IHC requests 
before cutting them and they will also often request a follow up H&E.  I have 
worked at another facility that would a 'level 1' and 'level 100' on all 
sentinel nodes without any IHC up front.

How many frozen levels? Seeing as we are serially sectioning the lymph node, we 
only do a single level.
How many permanent section Levels? All remnants are submitted after frozen, 
again we only perform the microtomy of a single level.
Any unstained ffpe sections saved or stained IHC? It is not uncommon for 
AE1/AE3 to be requested which we then cut the IHC request followed by an 
additional recut H&E.
