Hello, We had Leica Bond Max instruments (2 Bonds) for about 5 years and made the move over the Ventana Ultras (3 Ultras) a couple years ago. We made the move initially for the speed and menu benefits versus the Leica, (Ultra is about 2x faster) but also saw a quality improvement once we tweaked the protocols a bit. I don't know about the difference between the XT and the Ultra because we never had the XT, but from the Leica to the Ultra the biggest improvement was speed. We also saw dirty H. Pylori staining at first with both Leica and Ventana but have cleaned it up dramatically with some processing and fixation changes. I don't know if the instrument has much to do with the staining quality as the changes in our process because the staining was improved using the same Ultra instrument. If you like I would be willing stain your tissue on our instruments to see if you do in fact have a bad instrument??? I can also share with you our protocols to see if that helps your staining issues. [Histonet] Ventana Ultra CD10
We are in the last stages of contract negotiations for purchasing two Ventana Benchmark Ultras. During this time period we have been optimizing our current inventory of approximately 110 antibodies and validating before our current instrumentation is removed within the next two weeks. My Ventana application specialist is unable to successfully optimize CD10 (Ventana clone SP67) on the Ventana Benchmark Ultra. He had me send slides to their applications/troubleshooting lab but they told us they won't start working on it until next week and then it would take about a week for them to try and optimize it. We are in an urgent rush to get this antibody optimized and validated within the next two weeks since it is heavily requested by our Hemepaths. Would anyone be willing to share their Ultra protocols with me? Has anyone had similar experiences with Ventana being unsuccessful and having to send their slides to their applications lab for work up? Thank you for your help. The Her2 CB11 clone of Ventana is FDA approved. In the package insert of the Her2 4B5 clone is stated, that the antibody was compared to CB11 and shows same or better performance. The CB11 clone was exchanged through 4B5. I think Barbara was referring to the approval for the ER, PR and Her2 antibodies being for use with the Benchmark XT stainers and that they aren't yet approved for use on the ULTRA stainers using newer formulated bulk reagents. It's funny because they both use the same detection kits and antibodies. The formulation for the bulk reagents is different and it is a new instrument, so I think that's why they need another approval from the FDA. The antibodies were already approved for use with the Benchmark XT stainers. We aren't running Ventana's new Dual ISH Her2 probe kit but we will be looking at it. It does seem to have some perks from the lab's point of view. The instrument does the entire ISH procedure/stain. No more having to mix the reagents for the pretreatment as with Abbott's Pathvysion Kit, repeated pipetting, 22 x 22 coverslips or rubber cement. It's even worse if you don't have a VP2000 pretreatment unit, then you're stuck doing FISH pretreatment by hand with coplin jars in waterbaths. You don't need a fluorescent scope to see the slide with Ventana's new kit and it would be easy for the pathologist to make sure the correct area is scored for Her2 amplification (as can be a problem with the pathologist circling the area of interest on an HE and the tech matching it up to another unstained serial section with Her2 FISH). I know a lot of labs use the Pathvysion kit off label and dilute the probe and change other things from the original FDA approved protocol. Then again, even deparaffinzing the slide using xylene is off label, since it was approved using Hemo-D substitute. I imagine it would be harder to go off label with Ventana's software. Hi Antoinette, We currently use Ventana's ER and PR on both XT and Ultra. From what I understand, the antibody dispenser is labeled IVD. We report all cases as being 'approved by the FDA for IVD use'. We do perform correlation/precision studies twice a year. For this study we run five cases on both the XT and an ultra and document that the result comes out the same. I have actually found that this is a compliance issue with Joint Commission. I can try to find the reference if you need it. It states that if you are running the same test on two different platforms, you must prove correlation. As far as the Her2 dual ISH, we are in the process of evaluating it now. I see it as a definite advantage that it is bright field, permanent, and has the potential to dramatically increase TAT. We are in the middle of a cost analysis. On the other hand, we currently run Pathvysion and I see that some literature calls FISH and pathvysion the 'gold standard'. This is the most highly scrutinized test we perform considering the CAP/ASCO guidelines. My gut also tells me...why take a chance with this when our current system is not 'broken'? I did find that CAP offers two mailings for profiency testing using bright field ISH. Let me know how you make out. Hope this helps. each of the 30 drawers essentially is it's own run. You don't have to wait for all 30 drawers to finish their runs before adding more slides. There are 2 reagents that are exclusive to the Ultra and they are a little more expensive. Ultra CC1 and Ultra LCS,(and ultra CC2, if you use CC2). The EZ Prep, Reaction buffer and SSC are the same reagents that you use on your XT. The software can be a little quirky sometimes. But, overall, we really like ours because of the continuous feed ability. I currently have one XT and 3 Ultras. I love the Ultras. There is no difference in reagent costs. The only thing that I can say remotely related is that if you keep both platforms, the XT and the ultras have separate CC1, CC2, and LCS. I am fairly certain the contents of the bottle are the same, but the label and part numbers are different. I love the continuous access. I have it set up where the three ultras are 'top loaded' with set antibodies every day. This eliminates bouncing things around all the time. Contact me if you would like anymore info. I recently got an Ultra about a month ago and doing test runs, I have really enjoyed the continuous feed as well but I have a question...if I deactivate a product from the Ultra's inventory list I notice it's still in my XT's inventory list. We like ours as well. It's not as continual as we originally thought to maximize its potential I would recommend placing frequently used antibodies on the wheel with the first run. As long as you have the antibody you want to stain for on the wheel you can add the slide anytime you want and it will run it. If the antibody is not on the wheel you have to set a landing zone in order to add it. This usually works out fine, however once the run that is already going reaches a certain point, you aren't able to set a landing zone and may have to wait awhile to load new slides. It is pretty cool that you can run FITC, FISH, and IHC at the same time (however we tend to put our EBER on overnight because it's a fairly long run). What we like about the Ultra is, that there's no need for cleaning runs between the runs and that you can take out the bulk and waste containers during the run. We don't use the continious loading, because it doesn't fit to our workplan and because we use various antibodies from run to run. The runs are faster, but more than 2 runs per 8 hour shift and 1 run over night are not possible for us. That's the same workload we did with the XT. There are windows during the run, when you can add new slides and reagens. If the needed reagens in on the instrument, you can add them any time, if there's enough place. But the run will last longer and you have to wait for a window of the longer run to get the reagens off the instrument. Ready stained slides can be taken out independently. But the free pads cannot be filled and runs started, until you have the possibility to add the right reagens. So we decided to stay with patch-workload and real go-away. We load our Ultras with the most commonly used antibodies each morning. So that minimizes the number of landing zones we need to use. Also, (if you run both XTs and Ultras) run your antibodies with the longest protocols on Ultra and that will shorten your XT runs.
