Haven't seen this personally.

At the risk of sounding boring and obvious, have you checked you don't have
a server mod interfering?

A follow up to this issue, i cannot seem to replicate it consistently, but
it is still occuring on at least dustbowl, steel and badwater.

Is it always the third point of dustbowl? What's your mp_winlimit set to 
(I think that's the command that does it).

Does steam tool not generate those test.so files anymore?

In the past:
Steam used to generate these

test1.so
test2.so
test3.so

and would refuse to run without them.

It used to prevent Steam tool from running if one or all were missing.
I used to have to delete the ClientRegistry.blob to get Steam to recreate
them
in the event that one or all were missing. I scripted checks for them and
now
they don't seem to exist anymore. Much better to deal with, anyway.
