I think I've found what I'm looking for, thanks for the responses.  My 
clan has very specific needs.  We run game nights every Friday and 
Saturday night and we're not big, we're lucky if we have eight show up 
on a game night, so we use bots a lot just to fill out numbers.

We run eight different mods, and every Thurs and Fri we vote for Fri 
night's game (which mod we are going to run) and on Sat for Sat night's 
game.  The only cost effective way we have to do this is host our own 
server as we require a lot of space, but not much bandwidth, and during 
the rest of the week we host a website with forums, stats and whatnot.

So we host this all on a business cable connection.  And while we do 
quite well with the servers, we do have very limited bandwidth, and file 
hosting just isn't an option.  We run quite a few custom maps for a 
couple of the mods we play, and hosting our own files for fast downloads 
isn't possible with the bandwidth that we have available - we are really 
pushing the envelope with what we do have.

Anyway, the tldr version is that we don't really need more than 1GB of 
bandwidth per month for fast downloads because during an average month

You also want a server that gives real 404 errors for file not found.
Otherwise if a file is missing the client will get a bogus file that has the
wrong CRC and be screwed forever.  The game will not attempt to download the
file again since it thinks it already has it.  And most users have no idea
how to correct this by deleting that file.

Sad that Valve can't fix this.  They should verify the CRC and re-download
if it is wrong.

I used the cheapest plan here: http://www.caratnetworks.com/vps.php
It's not Web hosting you just get a shell so you apt-get install apache2 
vsftpd and configure yourself, if you know your way around linux it's a 
really good deal... I usually got speeds between 1MB/s at peak times and 
5MB/s at night.

back when we use to run a server, I had a fastdl server off my Dreamhost
account (it was fast enough for fastdl).
I had a shell script that ran once a day (cron job) that fetched the latest
updates from the main server for mirroring.
DH is unlimited space and bandwidth and for me, it costs less than 8 bucks
(give or take...I usually pay once every few years)
