Again, no this is not an sv_region bug.  It's because there are more people
wanting to play in that region than there are server slots in that region

A more correct answer would be that you can use cheats in a game that wasn't
created via a lobby. So like Brent said load the map locally, or connect to
an empty dedicated server through console.

Only thing thats nice is that you dont need sv_cheats to use the
mm_forced_dedicated_blabla server.. thats the only thing that works like a
charm.

Also sv_region 3 is set in my config, i would assume this would be "europe
people can join" but its all filled up with u.s. people.
Not that i have a problem with this, but i would assume u.s. people would
want to play on a server closer to home (lower ping)

For whatever reason, Valve patched out cheats in the demo shortly after it
was released. The only way to have cheats on is to just load a map locally.

That is correct, it is working as intended. If you wish to use sv_cheats you
should be running a local server at home. Though I'm unfamiliar with any
official word as to why this was implemented, I assume it was to improve the
quality of the match making service for L4D2. It is to weed out non-quality
sv_cheats running servers.
