Mysterious Question Mark (?) Appears Sometimes

For the last couple of weeks I've started to notice that, when I open 
certain files, that a *question mark appears* in the banner that runs 
across the top of the window. Other than appearing in those spots, it 
doesn't seem to do anything.

What causes this? Is it anything to worry about? And, how do I get rid of 
it?  Thanks.
