I am not able to repro the issue on acs 4.5 using simulator based on the steps 
mentioned in comment comment - 25/Feb/14 09:08. In the logs there is no 
StopCommand getting handled by HA-Worker threads which indicates that vmsync is 
not generating one.

In acs 4.3 and onwards, vmsync design was changed. This issue is fixed as part 
of that.

After a little investigation, we have some facts. Error stopping the machine is 
always associated with an entry in the logs of the content:

After analyzing the logs of the database:

vm_instance.update_count = 54
633 Query UPDATE vm_instance SET vm_instance.update_time='2014-07-24 
15:00:34', vm_instance.update_count=vm_instance.update_count+1, 
vm_instance.state='Running', vm_instance.host_id=2, vm_instance.pod_id=1 WHERE 
vm_instance.id = 10  AND vm_instance.state = 'Stopping'  AND

As can be seen ACS tries to update the entry twice but twice looking 
vm_instance.update_count field with a value of 54 
So the first update causes the counter vm_instance.update_count increased to 55 
so once the second SQL Update command executed incorrectly.

My guess is that the situation is related to a race condition in the HA-Worker 
threads, which by default is 5 

In the test environment, we try to change ha.workers value to 1 and we will see 
if the situation repeats itself.

For us the way to reproduce it would be to run your XEN cluster near capacity.
