revise continuum forms using ww:form

The current Continuum theme is 'default', instead of the actual default of
xhtml_css. Since we have a full copy of the theme including validation, we are
not able to obtain updated bugfixes from new releases. One such instance is the
incorrect clearing of error messages on the forms where the first element is
not a table row.
We need to:
* assess what customisations we actually need
* possibly switch back to the default theme and add just those
customisations
* remove some of the extra table tags in the jsps
* check for valid XHTML on all pages.
