I don't understand why you were unable to reproduce my results? The only difference between the steps you followed and the steps I follow is that it appears that you are (1) not using bullets, and (2) you are using an outline number and going to the 3rd level (6.a.iii). Select Continue with previous numbering. If this produces a heading number then issue has been solved. Note that a bulleted 'numbering' scheme w/o outline was used. When a bulleted 'numbering' scheme is used then it doesn't appear that the number mode gets reset. I have not tried this with a numbered outline.
