[iText-questions] iText in action 2nd edition - Chapter 9 related questions

Hi,
Thanks for writing a very comprehensive book on iText API(iText in Action 
- 2nd edition).
I have a question - i am searching for a example/sample where i can have a 
html representation of a form which can have Javascript valdiations(client 
side) and server validations and it can be pre-filled by some values from 
Database and later it can be saved or submitted to server.

I am into Chapter 9 - but these are small servlets which seems to be 
integrated for my purpose.
I need setup with MySQL - what is the entry point - where to start - i am 
not able to work thru FDF.

Needs some more help - can you tell me which examples in chapeter 9 or 
somewhere are using the DB? what all changes i need to do to make the 
MySQL DB to work? I have already made the entire chapter 9 code workable 
in my Eclipse.

Also while working with the FDF example in firefox and Chrome and other 
browsers - my Adobe Reader is not opening the generated FDF file though 
the same can be opened using a text editor and it contains all the 
required information. any help regarding this?
