STEPS TO REPRODUCE
1. Use Firefox to launch the website
2. Go to home page
3. Click on "Get Quotes"

OBSERVED RESULT
Only brought option for First name of Insurer

EXPECTED RESULT

It should bring both the First and Last name option

if vlc running and I open chrome it happens more often. I installed Kde Neon
user LTS, had same issue, now in mint kde, I have back dated framework 5.28 and
