If you press the reload button it goes back to normal. This happens every time you switch to the new image tab. In the brush editor, if you look at Pattern -> Options, you see a large change in the Horizontal and Verical Offset slider values when you reload it. Still, here are my observations: What a weird bug, but I can confirm it on the 4.2.9 and 4.1.7 appimages on Debian. I was trying a couple of other brushes as well: The "h) Chalk Grainy" and "h) Chalk Soft" seem to have it as well — I can't paint as lightly on the second image as I can on the first one — and more brushes might be affected as well, but it's hard to tell with some of them. It happens regardless of whether I have "Temporarily Save Tweaks to Presets" enabled or not, or if the images are displayed in windowed or tabbed mode, or whether I keep the first image open or not before I create the second one.
