I can still reliably reproduce it with Plasma 5.17.90 (and KDE Frameworks
5.66.0, Qt 5.14.1).

It seems to happen especially if I select Logout several times in a row (and
cancel it). Just now it didn't appear in the taskbar the first time, but did
appear when I cancelled and choose Logout in the menu again just afterwards.

I was able to reproduce this a few releases ago, but it's stopped happening to

I think you need an output when it bugs-out. Being unpredictable/random, that's

the second is for konsole. 

When you got that output, it was appearing in the taskmanager, correct?

ksmserver-logout-greeter looks to be doing everything fine.
