Title:
  Kernel 3.13 fail to boot with LSI SAS1068E (Dell SAS 6/iR)

Bug description:
  We have recently upgraded an Dell R300 server to Trusty (was running
  fine in precise), and after upgrade it fail to boot.

  It is an issue with the SAS controller during the initilisation. It
  fail to detect the disk, we have the following error in console log:

  [   36.539955] scsi4: error handler thread failed to spawn, error = -12
  [   36.552694] mptsas: ioc0: WARNING - Unable to register controller with 
SCSI subsystem

  After this error, initramfs drop to a shell complaining that rootfs is
  not found. No disk is seen at all (cat /proc/partition only show sr0 -
  cdrom drive).

  We have this issue with two different server (both R300, both Dell SAS
  6/iR controller and same hardware).

  We don't have this issue with another Dell server (R310, Dell PERC
  H200).

  We also tester with old kernel (generic, 3.2.0-58.88), it is working.

  Those server need a greater rootdelay (probably #579572), so we have
  rootdelay=45. If we remove rootdelay=45, then disk are correctly
  recognized ! (but few second too late, initramfs dropped to a shell.
  Pressing control-D resume normal boot)

  So the issue is that with the (mandatory) rootdelay greater that 30
  (default value I think), the disk are not detected due to the error
  shown above. This is a regression since those server worked in precise
  (and work with precise old kernel).

Hi folks,

I confirm that the drivers are not present in the server builds located
here: http://cdimage.ubuntu.com/ubuntu-server/daily/current/.

Please can you help find out the reason?

The drivers are there in the client builds. Our test guys confirmed this
by opening up the initrd in the ISO. We see the following contents in
the initrd:

  missing from the UEFI based alternate installer for server builds. The
  ask is to include all hyper-v drivers in to the alternate server

Also, do you have a prior version of Ubuntu on the system that boots?  If so, 
can you install the 3.13.16.36 kernel and see if it also has the issue?  It can 
be downloaded from:

If 3.13.0-16.36 does in fact boot, can you then try 3.13.0-17.37 and confirm it 
does not boot?  It can be downloaded from:

For both these kernels, you need to install both the linux-image and
linux-image-extra .deb packages.

I started a kernel bisect between v3.6final and v3.7-rc1. The kernel
bisect will require testing of about 13 test kernels.

I built the first test kernel, up to the following commit:
24d7b40a60cf19008334bcbcbd98da374d4d9c64

The test kernel can be downloaded from:

Can you test that kernel and report back if it has the bug or not. I

  1. boot into the system
  2. connect to wireless AP (if any) via the indicator on Desktop (upper-right 
hand side)
  3. if connected, make sure we could surf on internet
  4. press the wireless hotkey, in this case, it is F12

  Expected result:
  1. the wireless will lose connection
  2. no AP on the scanning list of the wireless desktop indicator
  3. wireless LED will change color (from white to orange)

  Actual result:
  nothing happened

  More information may help:
  1. "rfkill list" shows the soft block was still "no" after the wireless 
hotkey is pressed
  2. "sudo showkey -k" has no response when the wireless hotkey is pressed

Bug description:
  We have recently upgraded an Dell R300 server to Trusty (was running
  fine in precise), and after upgrade it fail to boot.

  It is an issue with the SAS controller during the initilisation. It
  fail to detect the disk, we have the following error in console log:

  [   36.539955] scsi4: error handler thread failed to spawn, error = -12
  [   36.552694] mptsas: ioc0: WARNING - Unable to register controller with 
SCSI subsystem

  After this error, initramfs drop to a shell complaining that rootfs is
  not found. No disk is seen at all (cat /proc/partition only show sr0 -
  cdrom drive).

  We have this issue with two different server (both R300, both Dell SAS
  6/iR controller and same hardware).

  We don't have this issue with another Dell server (R310, Dell PERC
  H200).

  We also tester with old kernel (generic, 3.2.0-58.88), it is working.

  Those server need a greater rootdelay (probably #579572), so we have
  rootdelay=45. If we remove rootdelay=45, then disk are correctly
  recognized ! (but few second too late, initramfs dropped to a shell.
  Pressing control-D resume normal boot)

  So the issue is that with the (mandatory) rootdelay greater that 30
  (default value I think), the disk are not detected due to the error
  shown above. This is a regression since those server worked in precise
  (and work with precise old kernel).

Hi,

It's acutually worse. Hibernate does *not* reliably work, more often than
not it cannot resume.
In addition grub needs 50! seconds to startup, allow me to navigate to the
3.13 kernel and then start to boot.
The boot process itself is fast as this computer has an ssd.

The downloaded 3.14 kernel only started up once and the following three
times it did end with a black screen and no prompt or any way to continue.

I also updated the BIOS to the newest 1.09 version from Lenovo. No
change.

I'm having the exact same problem, Internet being painfully unstable and
slow.  Running Ubuntu 13.10 amd64 and driver r8169 on 1000Mb/s. Just
switched from Windows 7, didn't have any problems there.

Hi,

After a few tests with the latest kernel (3.14.0-031400rc6-generic
#201403100035 SMP Mon Mar 10 04:36:54 UTC 2014 x86_64 x86_64 x86_64
GNU/Linux) it is far better, but not enough!

Basically, the resume work, but a kernel thread (kworker) is eating 100% of 1 
CPU core.
Time to time, it stop working for, something like less than 1s. I was able to 
launch htop under an X terminal, to see that.
The time this worker thread work, is not constant, something between 10s to 
2min. During this time X freeze completely, and all others programs with it. 
During the clean reboot ('sudo reboot" in my X terminal), it happend too. The 
events are still monitored during that time. Even if the screen was frozen, I 
could type on the keybord or move the mouse. Just have to wait the worker 
thread to let others programs to work a little.

I tried to look at the logs, but I didn't found any evidence which may permit 
me to better know what was going on.
The only error in /var/log/pm-suspend.log is:

Running hook /usr/lib/pm-utils/sleep.d/60_wpa_supplicant resume suspend:

I don't know if it is related.

You may notice that I have an another bug on this laptop. I can't use
the proprietary WiFi driver (which is installed by default). This driver
(firmware ? crashed with an "invalid instruction" error. It seems to not
be compatible with an "AMD Athlon(tm) 64 X2 Dual-Core Processor TK-55"
(on GNU/Linux only, I tried Win8.1 on this laptop, the driver worked
fine). That is why, I only use the ethernet connexion,with the FOSS
driver. Again, I don't know if this information can help too.

I will retry with the stock kernel, maybe it's the same, I just didn't
wait 2min before performing a halt/boot. There is no hardware reset on
laptops :-(

Sorry I didn't reply ... or very late!

Since I had this bug, I switched to the fglrx driver.
I have tested Ubuntu 13.10 a little bit, a few days, with the mesa driver, and 
I didn't get it again.
I have updated my BIOS too, but there was no change, anyway, I don't have 
anymore an Ubuntu 12.04.0 installed, I now run the 3.11 kernel on my Ubuntu 
12.04.
I will start soon to test Ubuntu 14.04 on my main computer (I currently only 
run it on my second one). But I think, this problem seems to be solved with 
newer kernel + mesa + Xorg stack.

  again. Most of the time Xorg stop to respond.

  I'm not sure what exactly is the cause of the problem, probably some
  instructions send to the GPU by compiz, which make the kernel GPU
  code to be locked. So my supposition is that the kernel code can have
  a deadlock situation.

  related to my problem.

  My ATI card is an "Sapphire Radeon HD 6850 1Go", and I use the free
  software driver. Using the previous version of Ubuntu I didn't have
  this error.

  I have almost no hope to have this bug fixed (too few infos, no one
  having it), but is there some method I can use to get more
  informations on this problem ? I will try a kernel debugging, but for
  a "random" bug, it will be more harder. Some day it crash, some it
  don't crash!

Bug description:
  Due to a bug in the Hyper-V host verion 2008R2, we need to use a
  slightly smaller  receive buffer size, otherwise the buffer will not
  be accepted by the legacy hosts. Without this fix, the network driver
  will not load while hosting Trusty on Server 2008R2. The following
  patch addresses the problem:

Hi Joseph,

The has been fixed in v3.14-rc6-trusty mainline kernel. But I guess,
14.04 LTS would ship with kernel v3.13. Can the fix be back-ported from
v3.14 to v3.13 before release?

Moreover, this bug relating to bluetooth has also been fixed in v3.14:

Bug description:
  I am using Ubuntu 13.10 and have an Ethernet connection to connect to
  the Internet.

  If I set my ISP username and password in router-cum-modem (ADSL)
  itself (i.e.; I get connected to internet automatically by just
  powering on my router); I don't get the information about `tx_bytes`,
  `tx_packets`, `rx_bytes` and `rx_packets` anymore.

  If I don't put the username and password in router; and instead use
  `sudo pppoeconf` to configure my connection by providing my ISP
  username and password;  I get the information about `tx_bytes`,
  `tx_packets`, `rx_bytes` and `rx_packets` as expected.

  It is probably because of this bug, that Gnome System Monitor doesn't
  update Network History as reported earlier:

Title:
  0a12:0001 Bluetooth Dongle (Cambridge Silicon Radio, Ltd) doesn't work
  after upgrading to Saucy

  After upgrading to Saucy, the bluetooth-indicator was no longer
  present in the taskbar. If I go to System Settings >> Bluetooth; it
  shows as "Bluetooth is disabled" and toggling the On|Off Switch

  I have installed the Blueman Device Manager to solve this issue and
  its bluetooth indicator is shown on the taskbar, but it doesn't
  function. However, toggling the On|Off switch in System Settings >>
  Bluetooth, grays out the icon of Blueman indicator in the taskbar.

  To debug the issue, I am following the instructions mentioned at

That return statement is called only when wait_for_completion_killable()
returned an error. That is, the caller received SIGKILL while waiting for
kthreadd to create a kernel thread.

That matches your bisection result because commit 786235ee changed to return to
the caller when the caller received SIGKILL in order to allow the OOM killer to
kill the process waiting for kthreadd to create a kernel thread.
The changelog which I expected for that commit is shown below.

[PATCH] kthread: Make kthread_create() killable.

Any user process callers of wait_for_completion() except global init process
might be chosen by the OOM killer while waiting for completion() call by some
other process which does memory allocation.

When such users are chosen by the OOM killer when they are waiting for
completion() in TASK_UNINTERRUPTIBLE, the system will be kept stressed
due to memory starvation because the OOM killer cannot kill such users.

kthread_create() is one of such users and this patch fixes the problem for
kthreadd by making kthread_create() killable.

I think there are two problems listed below.

  (a) Somebody is sending SIGKILL to the caller of kthread_create().

Somebody is "systemd" waited for timeout?
The caller is "PID: 9847 Comm: systemd-udevd" ?

  (b) Error handling of the caller of kthread_create() is wrong.

mptsas_probe() calls mptsas_remove() when
scsi_host_alloc() returned NULL due to receiving SIGKILL.

But mptsas_remove() assumes that "ioc->sh = sh;" was already called
with sh != NULL which means scsi_host_alloc() did not return NULL.

scsi_host_alloc() can return NULL when kzalloc() returned NULL.
In other words, the caller of scsi_host_alloc() must be prepared for
scsi_host_alloc() returning NULL even if the caller did not receive
SIGKILL while waiting for kthreadd to create a kernel thread.

Therefore, I don't think reverting commit 786235ee is appropriate because
the problem will again happen when kzalloc() in scsi_host_alloc() fails.

Title:
  Kernel 3.13 fail to boot with LSI SAS1068E (Dell SAS 6/iR)

Bug description:
  We have recently upgraded an Dell R300 server to Trusty (was running
  fine in precise), and after upgrade it fail to boot.

  It is an issue with the SAS controller during the initilisation. It
  fail to detect the disk, we have the following error in console log:

  [   36.552694] mptsas: ioc0: WARNING - Unable to register controller with 
SCSI subsystem

  After this error, initramfs drop to a shell complaining that rootfs is
  not found. No disk is seen at all (cat /proc/partition only show sr0 -
  cdrom drive).

  We have this issue with two different server (both R300, both Dell SAS
  6/iR controller and same hardware).

  We don't have this issue with another Dell server (R310, Dell PERC
  H200).

  We also tester with old kernel (generic, 3.2.0-58.88), it is working.

  Those server need a greater rootdelay (probably #579572), so we have
  rootdelay=45. If we remove rootdelay=45, then disk are correctly
  recognized ! (but few second too late, initramfs dropped to a shell.
  Pressing control-D resume normal boot)

  So the issue is that with the (mandatory) rootdelay greater that 30
  (default value I think), the disk are not detected due to the error
  shown above. This is a regression since those server worked in precise
  (and work with precise old kernel).

I've attached a picture of the log as described in the link you gave me.

Regarding installing the newer kernel on my installed system: It should
be easily reversible if it doesn't work, right? As I use this computer
daily, I don't want to break my current install.  Ubuntu 12.04 was pre-
installed by DELL if that matters

Yes, I can login as a guest then log out and login to my account and the
keyboard works.  It seems to only happen on boot, and when I click the
laguage icon "EN" it works.  This also seems to be the issue on another
laptop of mine I just installed 14.04 on.  So I'm guessing this is a
bug, but I don't know much.  Not a big deal, just an inconvenience.

Title:
  Kernel 3.13 fail to boot with LSI SAS1068E (Dell SAS 6/iR)

Bug description:
  We have recently upgraded an Dell R300 server to Trusty (was running
  fine in precise), and after upgrade it fail to boot.

  It is an issue with the SAS controller during the initilisation. It
  fail to detect the disk, we have the following error in console log:

  [   36.539955] scsi4: error handler thread failed to spawn, error = -12
  [   36.552694] mptsas: ioc0: WARNING - Unable to register controller with 
SCSI subsystem

  After this error, initramfs drop to a shell complaining that rootfs is
  not found. No disk is seen at all (cat /proc/partition only show sr0 -
  cdrom drive).

  We have this issue with two different server (both R300, both Dell SAS
  6/iR controller and same hardware).

  We don't have this issue with another Dell server (R310, Dell PERC
  H200).

  We also tester with old kernel (generic, 3.2.0-58.88), it is working.

  Those server need a greater rootdelay (probably #579572), so we have
  rootdelay=45. If we remove rootdelay=45, then disk are correctly
  recognized ! (but few second too late, initramfs dropped to a shell.
  Pressing control-D resume normal boot)

  So the issue is that with the (mandatory) rootdelay greater that 30
  (default value I think), the disk are not detected due to the error
  shown above. This is a regression since those server worked in precise
  (and work with precise old kernel).

  This has happened for the first time now, all previous cycles had no
  problems. Will update report with info on frequency of issue. Not
  working means that laptop shows no response to keypresses or touchpad
  use. No response to hotkeys either (i.e. Fn-x presses). Volume rocker
  at the side of the laptop doesn't work either. However, touchscreen
  and digitizer continue to work.

  WORKAROUND: A second suspend-resume fixes the issue. I'm attaching a
  dmesg log from after the second resume.

- LP: #1281620

According to the commit message for "ioat: fix tasklet tear down" it is
that net_dma commit which activates the bug. Please include the

  doesn't switch colors

Bug description:
  I'm using Lubuntu 11.10 Oneiric Ocelot. My wifi chip is supported from
  ath5k drivers. The led button blinks between red and blue light and
  its pressure does anything.

  WORKAROUND: I tried to type:
  echo none > "/sys/class/leds/ath5k-phy0::tx/trigger"
  echo phy0radio > "/sys/class/leds/ath5k-phy0::rx/trigger"

  and in this way I can control the color of the LED. But the problem is
  that the colors are reversed: it should be red for wifi off and blue
  for wifi on, but it is rather the opposite!
