Title:
Kernel upgrade to linux-image-3.16.0-28-generic breaks my external
  keyboard

Status in linux package in Ubuntu:
  Confirmed

Bug description:
I have an X1 Carbon Second Generation. With linux-
image-3.16.0-25-generic, everything works fine. With linux-
image-3.16.0-28-generic, if I boot with an external keyboard attached,
the keyboard generates keypress events only until I close my laptop
lid. After closing the lid, presses on the USB keyboard generate no
events. This state of affairs persists even after opening the laptop
  lid again.

The problem appears to be in the kernel somewhere. Booting with the
previous kernel using the GRUB menu option restores correct behavior.
Switching to a non-X VT and using evtest indicates that no events are
being delivered to userland. dmesg doesn't report any unusual messages
  across lid close and reopen.

  Repro:

  1) Boot with laptop lid open and external USB keyboard attached
  2) Log into X
  3) Type using external keyboard: note that keypresses are received
  4) Close laptop lid (using external monitor)
  5) Type on external keyboard: does nothing
  6) Open laptop lid
  7) Type on external keyboard: does nothing

Here is the requested metadata. Note that I'm generating this
information on a boot of the old kernel.

power cycling Emerson EM229 headset caused phone to reboot

Status in bluez package in Ubuntu:
  Confirmed

Bug description:
While sanity testing krillin rtm image 139, I paired a bluetooth
headset. I made a call with it, hung up, then turned the headset off.
I noticed this caused the BT indicator icon to invert, which is a nice
way to indicate whether something is connected. So, I turned the
device back on, waited for the icon to invert, turned the device off,
waited, and instead of inverting the icon... the phone rebooted.

Not sure what exactly failed here, like which component, but it's
definitely not good when a paired headset can cause the phone to
  spontaneously reboot.

[Kernel-packages] [Bug 1387949] Re: power cycling Emerson EM229 headset caused phone to reboot

2014-12-11 Thread Selene Scriven
I finally managed to trigger this bug again. The last_kmsg file is
attached, and confirms it is indeed a kernel panic. The traceback looks
promising too.

[Kernel-packages] [Bug 24776] [NEW] ipw2100 - Fatal interrupt. Scheduling firmware restart.

2014-12-11 Thread Launchpad Bug Tracker
You have been subscribed to a public bug:

/ ATTENTION
I believe this issue is specific to the error that a chipset that is not
802.11G capable will return when it picks up an a beacon from a router that is
not mixed-mode, but 802.11G specific. Please verify. Remove mixed-mode from
your G routers, and see if you can get this repeated. Then add back mix mode
and see if it goes away and if you're connected with 802.11B.
/

[ Current Status ]

The ipw2100 driver was last updated on 2007-01-31. That tarball release
contained an issues file that described this particular bug:

Firmware restart is detected randomly
--

After running one or two days, Intel PRO/Wireless 2100 Network Connection
driver will only occasionally restart the firmware.
ipw2100: Fatal interrupt. Scheduling firmware restart

To resolve this problem, the system must be rebooted.

Defect #329

[ Original Description ]
Since I've upgrade to Breezy, ipw2100 driver no longer works and in dmesg I've
got plenty
Fatal interrupt. Scheduling firmware restart.

I'm always able to do an iwlist :

% sudo iwlist eth1 scanning
eth1 Scan completed :
...
  Cell 02 - Address: 00:07:CB:51:65:E0
ESSID:gaspard
Protocol:IEEE 802.11b
Mode:Master
Channel:11
Encryption key:on
Bit Rate:11 Mb/s
Extra: Rates (Mb/s): 1 2 5.5 11
Quality=92/100  Signal level=-36 dBm
Extra: Last beacon: 31ms ago

But the card no longer register. It stays unassociated whatever I've
tried :

% sudo iwconfig eth1 essid gaspard key open XX mode auto
% sudo iwconfig eth1
eth1 unassociated ESSID:gaspard Nickname:ipw2100
  Mode:Managed Channel=0 Access Point: 00:00:00:00:00:00
  Bit Rate=0 kb/s Tx-Power:off
  Retry min limit:7 RTS thr:off Fragment thr:off
  Encryption key:4D7E-7ED3-1D Security mode:open
  Power Management:off
  Link Quality:0 Signal level:0 Noise level:0
Rx invalid nwid:0 Rx invalid crypt:0 Rx invalid frag:0
  Tx excessive retries:0 Invalid misc:0 Missed beacon:0

Sometimes, it works for a short time and after half an hour or an hour, it crash
again.

I've tried to upgrade ipw2100 and the firmware. No way.

not mixed-mode, but 802.11G specific. Please verify. Remove mixed-mode from
your G routers, and see if you can get this repeated. Then add back mix mode
and see if it goes away and if you're connected with 802.11B.

  [ Current Status ]

The ipw2100 driver was last updated on 2007-01-31. That tarball
release contained an issues file that described this particular bug:

  Firmware restart is detected randomly
  --

After running one or two days, Intel PRO/Wireless 2100 Network Connection
driver will only occasionally restart the firmware.
ipw2100: Fatal interrupt. Scheduling firmware restart

To resolve this problem, the system must be rebooted.

  Defect #329

  [ Original Description ]
Since I've upgrade to Breezy, ipw2100 driver no longer works and in dmesg I've
  got plenty
  Fatal interrupt. Scheduling firmware restart.

I'm always able to do an iwlist :

  % sudo iwlist eth1 scanning
  eth1 Scan completed :
  ...
Cell 02 - Address: 00:07:CB:51:65:E0
  ESSID:gaspard
  Protocol:IEEE 802.11b
  Mode:Master
  Channel:11
  Encryption key:on
  Bit Rate:11 Mb/s
  Extra: Rates (Mb/s): 1 2 5.5 11
  Quality=92/100  Signal level=-36 dBm
  Extra: Last beacon: 31ms ago

But the card no longer register. It stays unassociated whatever I've
  tried :

% sudo iwconfig eth1 essid gaspard key open XX mode auto
  % sudo iwconfig eth1
  eth1 unassociated ESSID:gaspard Nickname:ipw2100
Mode:Managed Channel=0 Access Point: 00:00:00:00:00:00
Bit Rate=0 kb/s Tx-Power:off
Retry min limit:7 RTS thr:off Fragment thr:off
Encryption key:4D7E-7ED3-1D Security mode:open
Power Management:off
Link Quality:0 Signal level:0 Noise level:0
Rx invalid nwid:0 Rx invalid crypt:0 Rx invalid frag:0
Tx excessive retries:0 Invalid misc:0 Missed beacon:0

Sometimes, it works for a short time and after half an hour or an hour, it
crash
  again.

[Kernel-packages] [Bug 1387949] Re: power cycling Emerson EM229 headset caused phone to reboot

2014-12-11 Thread Ricardo Salveti
Nice, thanks for the trace, it should hopefully be enough to indicate
the problem (which is clearly in the krillin bluetooth driver).

** Also affects: canonical-devices-system-image

** Changed in: bluez (Ubuntu)

power cycling Emerson EM229 headset caused phone to reboot

Status in the base for Ubuntu mobile products:
  New
Status in bluez package in Ubuntu:

I don't see any relevant .crash files, and all I see in syslog from
  that time is this:

[Kernel-packages] [Bug 1401741] [NEW] rtl8188ee wireless driver high packet loss

2014-12-11 Thread Bryan Quigley
Public bug reported:

I've seen this issue since trusty, very bad packet loss, varrying in ping
from 10%-45% to the gatewway. This is a toshiba c55d laptop.

Already tried mainline (3.18.0) with no noticable improvement.

Previously reported here:
https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1258870

AP is a MI424WR-GEN3I from Verizon with WPA2 running in Compatibility
Mode(802.11b/g/n)
No other devices on network have ever had this issue, in fact any loss of ICMP
packets almost never occurs on other devices from same location.

I will be trying the Realtek (from their website) and
https://github.com/FreedomBen/rtl8188ce-linux-driver next. Any other
ideas/suggestions welcome.

Impact: Under certain workloads a BUG in the xen-netfront driver is
  getting triggered.

Fix: Remove the offending BUG, and a related BUG which would have also
been triggered by the same conditions.

Test Case: This bug has only been seen under certain workloads
involving haproxy and has proved challenging to reproduce. The fix has
been verified by an affected user, with the BUG replaced by a WARN to
ensure that the conditions which would have triggered the bug were
  actually encountered.

Regression Potential: The fix has been verified and acked upstream.
Removing the BUGs will not affect cases which were not crashing
previously, and cases which were crashing can't really be made much
  worse.

  ---

I run ubuntu servers 13.04, 13.10, and 14.04 on the AWS cloud.
Whenever I install haproxy and nginx on the same server, after hitting
it for a while, a kernel panic is caused. However, the kernel panic
does not show up in ubuntu 12.04. The kernel panic is pasted at the
  end of the attachment.

  SRU Justification:

Impact: the card reader does not work on this system after s3.
Fix: cherry pick a simple patchset only for the certain driver module.
Testcase: already tested in OEM projects

  Before suspend
  [ 12.896116] rtsx_pci :03:00.0: irq 47 for MSI/MSI-X
  [ 12.896128] rtsx_pci :03:00.0: rtsx_pci_acquire_irq: pcr-msi_en = 1,
pci-irq = 47
  After resume
  [ 89.354583] rtsx_pci :03:00.0: no hotplug settings from platform

  rmmod rtsx_pci
  modprobe rtsx_pci
  doesn't work

  Reboot

  [ 12.358103] rtsx_pci :03:00.0: rtsx_pci_acquire_irq: pcr-msi_en = 1,
pci-irq = 47
  [ 12.455971] rtsx_pci: probe of :03:00.0 failed with error -110

  Then driver cannot be loaded

  * Card reader should be found in lspci output, SD card should be mounted
automatically

  Actual result:
  * Nothing happened, the card was not detected at all
  * On step 4, the card reader works, if rebooted again, it will be gone.

Status in linux package in Ubuntu:

Bug description:
Ubuntu 14.04 on ThinkPad T440s in clamshell mode using external
  monitor:

The mouse periodically (not always) starts lagging dramatically. It's
difficult to position it when this happens, as I'll move the mouse and
the pointer will react a later, sometimes a long as a second, and with
little precision. Virtually unusable when it gets this way.

I've checked system monitor for runaway processes, and nothing is
  hogging the CPU.

The mouse is a Microsoft wireless (optical). This also occurred with a
Logitech wireless mouse.

Title:
SD card is spontaneously unmounted when files are copied to it

Status in linux package in Ubuntu:
  Expired

Bug description:
I have this problem in Ubuntu Desktop 7.04. Here's how it happens:

  - I insert a 2 GB SanDisk SD card into my laptop's built-in card reader
  - It appears a sa NEW VOLUME item on the GNOME desktop
  - I click the item and see the files on the card
  - With dragging-and-dropping, I start copying a few 30 MB files to the card
  - In the middle of copying process I get the message baloon at the bottom
right saying that device was unplugged, and that's a bad thing to do. That I
should use safe removal insted. BUT: I did not touch the card.

So, what I do is copying files one by one, unmounting the card after
each file, and mounting it back. Then, eventuallly, I manage to really
transfer the files to the card. However, often this also does not
work. A file does not appear on the card after the safe removal.

Bug description:
  Hello,

my external USB Sound Interface is not supported from the generic kernel.
I know the Chip is very similar to the Focusrite Scarlett 2i2 which works
very fine. Defining the USB Device in the quirks-table as generic audio device
fixed this.

Is it possible to add this lines in the next patch somewhere in
sound/usb/quirks-table.h?
When there is a kernel update, I have to add it and compile it everytime on
my own to get this device woking.

[Kernel-packages] [Bug 1401439] Re: Could not resume after suspend. Always shows mei_me error.

2014-12-11 Thread ben sen
I'm using Xubuntu 14.04 as my default OS in work and I'm afraid of
loosing my personal data while trying new kernel. If my laptop crashes,
I will not have enough time to rebuild it.

And also I do not know how to reproduce it, it happened yesterday again
but it is not dependent on anything (of course it is, but the
dependencies of the problem are not observable for me). It is happening
randomly for me.

Is there anything I can help whit this?
