  In trying to build a zoneminder server, I think I stumbled on a kernel
  bug. It  seems to have snuck in sometime between 16.04 desktop
  promoted to server, and 17.04 server promoted to desktop.

  The suggestion is that the popular Renesas / NEC - µPD720202 may need
  "quirk" in the kernel.

  Timeline:

  (Scene 1)

  Loaded 16.04 Ubuntu Desktop
  Promoted Desktop to Studio
  Promoted Desktop to Server
  Added Zoneminder
  Needed more cams, added Startech  PEXUSB3S44V Renesas / NEC - µPD720202 based 
card for more USB channels.
  Everything worked quite well. Some errors in ZM but no DMESG as far as I 
recall.

  (Scene 2)
  Wiped the drive.
  Loaded 17.04 Server
  Now getting messages in DMESG complaining of thousands of errors:
  [96415.044917] handle_tx_event: 516 callbacks suppressed
  [96415.044924] xhci_hcd :0c:00.0: WARN Successful completion on short TX: 
needs XHCI_TRUST_TX_LENGTH quirk?

  Messages suggest "quirk" is the solution.
  a)Researched usb 3.0 "quirk'
  b)Noticed it's really a "thing"
  c)Couldn't find recipe for patching kernel for NEC/RenasaSs "quirk" fix.

  [EDIT]: Note C920 webcam is USB 2.0.

  Anecdotes suggest Nec/Renasas firmware is to blame.
  a) Kernel indicates 2024 firmaware
  b) Startech website has 2026 firmware.
  c) VERY IMPORTANT! WindowCentric GUI instructions do not translate between 
ASCII and HEX. So if your Startech is on channels 9, 10, 11, and 12 from 
perspective of Windows, you must translate into 09, 0A, 0B, and 0C when editing 
runfile for effective firmware update of all 4 controllers. Yes, for the 4 
channel controller, count them: 4.
  d) Firmware updated.

  (Scene 4)
  Rebooted with new Startech/Nec/Renases 2026 firmware.
  SAME OLD PLETHORA OF ERRORS

  (Scene 5)
  I give up.

  It may well be worth noting that there are no errors from DMESG until
  some seconds after the first webcam is attached to the Startech.

  And I can't overemphasize the fact that I don't think I had any DMESG
  errors under Desktop 16.04

  I think the file attached represents DMESG immediately before the
  errors start going nuts. My theory is that ZM does lazy polls on
  missing cams but once it gets 'hold if the cam and sucking data the
  quirk bug surfaces.

Bug description:
  In trying to build a zoneminder server, I think I stumbled on a kernel
  bug. It  seems to have snuck in sometime between 16.04 desktop
  promoted to server, and 17.04 server promoted to desktop.

  The suggestion is that the very popular Nec/Renasas 27xxx2 may need
  "quirk" in the kernel.

  Timeline:

  
  (Scene 1)

  Loaded 16.04 Ubuntu Desktop
  Promoted Desktop to Studio
  Promoted Desktop to Server
  Added Zoneminder
  Needed more cams, added Startech  PEXUSB3S44V NEC/Renesasa 27XXX2 based card 
for more USB channels.
  Everything worked quite well. Some errors in ZM but no DMESG as far as I 
recall. 

  (Scene 2)
  Wiped the drive. 
  Loaded 17.04 Server
  Now getting messages in DMESG complaining of thousands of errors:

needs XHCI_TRUST_TX_LENGTH quirk?

  
  Messages suggest "quirk" is the solution.
  a)Researched usb 3.0 "quirk'
  b)Noticed it's really a "thing" 
  c)Couldn't find recipe for patching kernel for NEC/Renases "quirk" fix. 

  (Scene 3)
  Anecdotes suggest Nec/Renasas firmware is to blame.
  a) Kernel indicates 2024 firmaware
  b) Startech website has 2026 firmware. 
  c) VERY IMPORTANT! WindowCentric GUI instructions do not translate between 
ASCII and HEX. So if your Startech is on channels 9, 10, 11, and 12 from 
perspective of Windows, you must translate into 09, 0A, 0B, and 0C when editing 
runfile for effective firmware update of all 4 controllers. Yes, for the 4 
channel controller, count them: 4. 
  d) Firmware updated. 

  (Scene 4) 
  Rebooted with new Startech/Nec/Renases 2026 firmware. 
  SAME OLD PLETHORA OF ERRORS

  (Scene 5)
  I give up. 

  It may well be worth noting that there are no errors from DMESG until some 
seconds after the first webcam is attached to the Startech.
   
  And I can't overemphasize the fact that I don't think I had any DMESG errors 
under Desktop 16.04 

  I think the file attached represents DMESG immediately before the
  errors start going nuts. My theory is that ZM does lazy polls on
  missing cams but once it gets 'hold if the cam and sucking data the
  quirk bug surfaces.

  Hi, after installing kernel 4.4.0-67 or later I cannot backup my Ubuntu VM's 
on Hyper-V.
  Microsoft Hyper-v 2012r2 Gen2 VMs

  See Attachment for what happens is immediately after backup starts I
  get an error.

  Eventually the kernel reports it has run out of memory and the machine
  just continuously scrolls errors message related to page allocation.

  When reseting the virtual machine no logs can be found of the problem.

For reference I've got this problem on Ubuntu 16.04 with the HWE kernel
installed on VM's as well. I'd gone a long time using LXC and KVM,
avoiding Hyper-V because of the VSS Read Only issue that took 2 years to
fix. Noticed that was marked as fixed and almost straight away on first
try of Hyper-V again hit this issue :(

Machine was on Dynamic memory but I've tried giving it 4GB of RAM fixed
as well. Currently it's only got Docker and Portainer pretty much
running on it and normally loads of free RAM.

Backups for machines start at 2am and the console I don't have an image
of but is showing the same messages people are saying on the 4.4 kernel
here.

actually AC 7265 has different firmware files than AC 7260. There are
even two files depending on your hardware revision. I can't tell which
one you have and therefore you may want to replace both:

  No input or re-pairing is possible after those timeout messages.
  Reloading the kernel firmware modules doesn't help either:

Hi

I have a new Dell PowerEdge R640 server a PERC H740P RAID Controller.

I tried to install the latest 16.04 LTS Ubuntu on it, but during
installation it failed to detect any disks. After pulling my hair out
while diagnosing this issue for hours I finally found this thread.

Can anyone confirm that the behavior I'm seeing (no disks showing up
during setup) is because of the lacking support?

If so, is there any way I can test this or to help to speed up the
release, as I need to setup this server with Ubuntu but basically I'm
stuck due to this.

I applied the patched /lib/firmware/intel/ibt-
hw-37.7.10-fw-1.80.2.3.d.bseq as mentioned by @7oby in comment #4 on
Ubuntu 16.04.3.  No Joy.  My control, is a newer:

[9.468054] iwlwifi :02:00.0: Detected Intel(R) Dual Band
Wireless AC 7265, REV=0x210

but the patched firmware seems to work just fine.  (I have rolled back
to the original, however.)  Hardware is Intel NUC NUC5i5RYH.

Bluetooth has been working fine with my primary bluetooth speaker since
I first installed 16.04 shortly after it was released.  As of sometime
in the last two weeks it sometimes works and sometimes doesn't.  The
problem appears after I have to reboot.  A few days ago, it wasn't
working and I just gave up and used a cable.  But then at some point
after another reboot it started working again.  Today it stopped again
(no updates during this whole time).

Gotta love intermittent HW-related problems. ;-)

Didn't understand what I am supposed to do, sorry.

The bug itself in my case is not entirely "can't work" - it is working
for some time and then stops working, needing restart of the connection
or (if it doesn't help) network-manager or (if it doesn't help either)
the notebook itself. It is very annoying to be in such "dualistic"
state: it is simultaneously working well and not working at all))

Solved. The problem with the sound was due to mount partitions.

  I just installed a new 14.04 server install on the following hardware:

  ASRock E3C224-4L Server/Workstation Board (Intel C224 chipset)
  Intel Xeon E3-1245V3 Quad Core LGA1150 CPU
  32GB of DDR3 ECC RAM
  LSI 9201-16i SAS/SATA HBA

  I am unable to power down the system without physically doing it with
  the button, or powering it off using the inbuild IPMI management.

  All of the following result in a reboot:

  sudo shutdown -h now
  sudo shutdown -p now
  sudo shutdown -P now
  sudo halt now
  sudo halt -fihp now
  sudo init 0

  I have tried acpi=force, acpi=off, acpi=noirq, noapic

  I tried the script from
  http://askubuntu.com/questions/13288...-of-a-shutdown as follows:

  That didn't help.

  I also set WOL off on the NIC that was plugged in (only one of the 4
  onboard NICs is connected at present, for testing) with sudo ethtool
  -s eth0 wol d and it didn't help, I've looked in the BIOS, and can't
  find a WOL section in there. To try to eliminate WOL, I also tried
  with neither the NIC in use for testing, nor the dedicated IPMI NIC
  connected, and it didn't help.

  I've updated my BIOS to the latest version.

  I've probably tried more that I've forgotten to list here - I've
  exhausted my options!

  I'm at a loose end now - no idea what to do next to further
  troubleshoot it, has anyone else had any similar problems on C224, or
  does anyone know what logs I should be inspecting, or how I can
  generate further debug logs?

Bug description:
  RTS5129 Card Reader has issues with rtsx_usb module on kernels >=3.16
  (since kmod-staging rts5129 module replaced with rtsx).

  Copying files from FAT32 SDHC memory card produces error messages in syslog:

  Also these errors indicate that copied files are corrupted - md5sum
  shows different checksums for copied file after each "insert card/copy
  file/check md5sum/eject card" cycle (actually you can skip "copy file"
  step and run "check md5sum" step directly with file on mounted memory
  card filesystem).

  Running sudo badblocks -n /dev/mmcblk0 produces same error messages
  and prints a lot of random block numbers.

   - dell inspiron 7720 laptop (a few different RAM modules and HDD drives used 
for testing);
   - Realtek Semiconductor Corp. RTS5129 Card Reader (ID 0bda:0129);

If this kernel still doesn't work, try latest mainline kernel with
"nvme_core.default_ps_max_latency_us=1500", do disable deepest power
state.

Bug description:
  Originally thought my issue was same as this:
  https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1678184
  but requested to report as separate bug

  System becomes unusable at seemingly random times but especially after
  resume from suspend due to disk 'disappearing' becoming inaccessible,
  with hundreds of I/O errors logged.

  After viewing the above bug report yesterday as a quick temporary fix I added 
kernel param, updated grub, etc with:
  GRUB_CMDLINE_LINUX_DEFAULT="nvme_core.default_ps_max_latency_us=0"

  System appears to have been stable for the last day, but is presumably
  using more power than it should.

Bug description:
  U-boot is now able to emulate UEFI and this is used by openSUSE to
  boot the raspberry pi [1].

  I would like to use this technique to load grub2.  However, according
  to [2] new versions of grub2 require a 32 bit kernel to be built with
  EFI stub enabled.

  I have successfully used this U-boot/grub2 combination to boot a
  generic aarch64 Ubuntu kernel on my pi3 [3].  It works well and would
  be a huge improvement if this could be brought to arm 32 bit.
