After this, the laptop doesn't recognise any device plugged in to the USB ports - plugging and unplugging any device I've tried into any of the USB ports produces no response, not even dmesg entries. Strangely this also applies to bluetooth - it no longer works once USB has dropped (possibly the module is hung of the bus internally). Once this has happened only a reboot fixes it; I've not managed to find any combination of module unload/reload or suspend cycles to reinitialise things correctly. All the way since 17.04 I have experienced random freezes, where the entire system would be unresponsive, I was unable to Ctrl+Alt+F1 to a different console, and I couldn't connect to it via ssh. After upgrading to 17.10 (and a fresh install of 17.10), and subsequently upgrading to 18.04, the issue intensified, appearing at least hourly, often quicker than that, and only a hard reset would let me access my system again. The issue is also present in Linux Mint 18.3. Freezes could happen at any time, regardless of usage (or just idling). After getting a tip that this might be caused by the WiFi card, I looked at the provided firmware from Intel's site, and compared the files to the files present in /lib/firmware/. The file provided by Intel for my specific card is iwlwifi-7260-14.ucode, but the files present in the folder were the numbers 7, 8, 9, 10, 12, 13, 16, and 17. Downloading the firmware from Intel, and adding the -14 file to the folder solved the issue of the screen freezing, and the issue has not appeared for the ten hours I've been running since applying the fix. I'm also getting the same error: "Bluetooth: hci0: last event is not cmd complete (0x0f)" The message always appears as Bluetooth audio starts to skip. Also the device keeps disconnecting and reconnecting. There is no message specific to when the device disconnects. Running "rmmod b43" always hangs. So far I have only tried it while connected to a wireless network. I immediately disconnect from the wireless network, so it seems module removal does begin. But the rmmod process never finishes and is unkillable. The hang seems indefinite, and a normal reboot is not possible, the rapid ctrl-alt-del systemd emergency reboot is impossible, and only a Magic SysRq reboot works. This worked in Ubuntu 17.10, but always fails in Ubuntu 18.04. (I was unloading the module before suspend due to occasional problems after resume. I tracked this down when wondering why suspend was hanging. Even running the "rmmod b43" on its own without suspend happening causes the hang. Right now I disabled that automatic unloading.) On Ubuntu 16.04 freshly restored from the Dell recovery image, I find that it's often difficult to get the trackpad to recover. I do find that the issue happens less frequently on Dell's build of Ubuntu 16.04 than on other linux distributions and versions, but it still happens enough to significantly impact my ability to do productive work. The issue does not occur when running Windows 10 on the same device. I suspect that the issue may be related to palm detection or misdetecting the user's palm as a gesture to enable scrolling mode, but have no proof for this. It seems that I'm often able to reproduce the issue by brushing the trackpad with my right palm. Public bug reported: I'm running Ubuntu on a Dell Inspiron 6400 with a 14e4:4328 "Dell Wireless 1500 Draft 802.11n WLAN Mini-Card". Running "rmmod b43" always hangs. So far I have only tried it while connected to a wireless network. I immediately disconnect from the wireless network, so it seems module removal does begin. But the rmmod process never finishes and is unkillable. The hang seems indefinite, and a normal reboot is not possible, the rapid ctrl-alt-del systemd emergency reboot is impossible, and only a Magic SysRq reboot works. This worked in Ubuntu 17.10, but always fails in Ubuntu 18.04. (I was unloading the module before suspend due to occasional problems after resume. I tracked this down when wondering why suspend was hanging. Even running the "rmmod b43" on its own without suspend happening causes the hang. Right now I disabled that automatic unloading.) When I try to attach an external monitor via an HDMI cable the monitor receives no signal. Similarly, when I use the Thunderbold dock and attach two monitors (one via HDMI, one with DisplayPort) only the monitor attached with DisplayPort works; the monitor attached with HDMI stays black. This made me think if this is really a bug in the software. I saw people describing this issue with Windows installed; the solution was to hit a special key combination.
