Note that I've found dozens of posts on the web that describe the same
symptom, and a subset of those arrive at the same conclusion of this
bug. I do agree that the license that the github repo holds indicates
that the files are made available for personal use, but perhaps we can
address that.

For my laptop, which is an x230 model 2325KZ5, I have:

  Bus 001 Device 004: ID 0a5c:21e6 Broadcom Corp. BCM20702 Bluetooth 4.0
[ThinkPad]

The file which I needed to use was brcm/BCM20702A1-0a5c-21e6.hcd as
noted above. That is the only file needed to fix my issue.

The post I linked to is for 0a5c:21e8 that I noted above.

We have just encountered this issue on a large deployment on one random
node, but it's not a Power8 machines -- just a regular HP DL360. Here's

Hmm. Does this problem not reoccur every time a new kernel package is
installed? Or does it happen only once when upgrading into Xenial?

I'm not sure we should remove the package from the repo, given I think
there are valid use cases for it, but perhaps we could make it so
installing the Xenial kernel removed it.

  Did an apt-get update && apt-get dist-upgrade. My /boot partition did
  fill up at some point but I was able to recover with autoremove.

  apport opened this bug so I'll send it anyway in case something in it
  is helpful. FWIW I've stopped evaluating zfs for the time being
  because it doesn't seem to work with ceph in containers so this isn't
  really a problem for me atm as long as my laptop still boots.

This is affecting an increasing set of users; for instance, it seems to
be the underlying problem in bug 1545238. My Thinkpad X220 is also
showing the same problem, which suggests multiple Intel graphics systems
may be affected.

  The problem has also been documented by somebody else in the Fedora

  This behaviour is 100% reproducible. I have asked the fuse-devel
  mailinglist for advice, but up to this point with no success:

  USB3 port yields slow performance, dmesg flooded with errors. Plugging
  on USB2 port, exact same situation otherwise, works well. This is
  reproducible 3 out of 4 attempts. Machine has 2 USB3 ports on the
  left, 2 USB2 ports on the right.

  Commit f95499c3030f ("n_tty: Don't wait for buffer work in read() loop")
  introduces a race window where a pty master can be signalled that the 
ptyslave was closed before all the data that the slave wrote is delivered.
  Commit f8747d4a466a ("tty: Fix pty master read() after slave closes") fixed 
theproblem in case of n_tty_read, but the problem still exists for 
n_tty_poll.This can be seen by running 'for ((i=0; i<100;i++));do ./test.py 
;done'where test.py is:

I haven't been able to reproduce the hang on the 3.18.1 kernel I am
using, but I can't easily reboot the machine many times as it's a
production server. I need to get a spare board to really test.

  I'm on an AWS EC2 m3.large instance with the official Ubuntu AMI ami-
  776d9700.

  After running for many days, the machine locked up with the below
  messages appearing on the console. The machine would respond to ping
  but not SSH or HTTP requests. The machine has one BTRFS volume which

  on top of one Amazon Elastic Block Store (EBS) device.

I can't use apport-collect from the terminal of that machine (lynx fails
the launchpad login dance). I'm attaching the saved .crash file I got by

  version, so it may be best built separately from the kernel packages.
  There is no consensus among other distributions: Fedora includes it in
  their kernel-tools package; SuSE and RHEL pull from the git repo and
  build as a separate cpupower or cpupowerutils package (similar to how
  cpufrequtils was done); Arch takes it from the kernel source and
  builds it along with other tools into separate packages.

  cpupower provides a number of things that cpufrequtils does not, and
  is invaluable in a server environment for tuning of power management.
  Of particular note is the ability to set perf_bias on Intel
  processors; I am not aware of another tool that can do this. It is
  also the focus of power utility development, and should be available
  on Ubuntu systems.
