This exact bug happens all the time for me on calls for more than around 2min.
The Bluetooth connection fail also happens during dialling - often on the first 
call after a connection is established.

The only way to re-establish a connection is to disable bluetooth and
then re-enable it, and the fastest way to ensure it works is to put the
phone in flight mode and then turn that off again.

  I was in a longer telephone call. All of a sudden the bluetooth
  connection got interrupted. At the time when the bluetooth connection
  got disconnected the following statement from bluetooth was logged:

(110)
  + following logs (see attached)

  The call itself still was active just the phone disconnected from my
  car.

  phone doesn't react at all and after a while the car system says that
  it failed to connect.

  Shutdown and Reboot failure on Dell XPS/nVidia 295.49/Ubuntu 12.04

  Sometimes "sudo reboot -h now" works, sometime not.

  Had the same problem on 11.10 so I didn't install nVidia driver on a
  clean 12.04 install but problem was still there. Installed the nVidia
  driver because it didn't make a difference.

  Tried stopping various unity processes using "system monitor" before a reboot 
(sudo reboot -h now) and this seems to work but it is terribly inefficient
  The unity processes stopped:
  unity-files-daemon
  unity-applications-daemon

  I then tried it using the ubuntu "shutdown" button after stopping the
  same processes and it also seems to have worked

  WORKAROUND: Reverted to non-mainline kernel "linux-headers-3.2.0-25
  -generic-pae" and removed all nvidia drivers. Operating on nouveau
  graphics drivers shutdown and Restart work perfectly.
