I just ran into a 12.04.03 client, and it does have the problem.

Ted Cabeen, thank you for your response. Would it be terribly
inconvenient to test if a 12.04.03 client works with your 12.04.03
server? This would help narrow down if the issue is client, or server.

Hmmm.  Hard to say.  The server was at 12.04.02 when I took over
management of the systems, and I upgraded the clients to 13.04 and the
server to 12.04.03 at about the same time, so it's hard to disentangle
which change caused the problem.  I certainly had 12.04.02/12.04.02
client/server systems running properly.

Bug description:
  After a recent upgrade on my clients to Ubuntu 13.04 latest and Ubuntu
  12.04.03 on the server, NFS4 clients began locking up with the errors
  below.  Processes accessing NFS would hang up and lock, and could not
  be killed or adjusted.  NFSv3 on the same server and client has been
  working fine.

  NFS4 was configured with clean defaults, no special options set.  No
  log entries corresponding with the problem were seen on the nfs
  server.

Would it be possible for you to test the latest upstream kernel? Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
v3.12 kernel[0].

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, 
please add the tag: 'kernel-unable-to-test-upstream'.
