Since the issue always happened when running a virtualbox virtual
machine I did some research in their issue tracker and I found a couple
of releted bugs (https://www.virtualbox.org/ticket/14006 and
https://www.virtualbox.org/ticket/13961). The proposed patch
(https://www.virtualbox.org/raw-attachment/ticket/13961/diff_smap_4) for
the virtualbox kernel modules solves the problem for me.

This is a new install, I could not test any previous kernel version. I 
installed the newest kernel version suggested (v4.0-rc6) but I could not boot 
to the desktop. I could boot only by selecting "recovery mode" but then 
failsafe X did not work.
As requested, I'm adding the 'kernel-unable-to-test-upstream' tag, but I don't 
know if I can mark the bug as confirmed since I'm the original reporter.
Is there anything more I can do?

Did this issue start happening after an update/upgrade?  Was there a
prior kernel version where you were not having this particular problem?

Would it be possible for you to test the latest upstream kernel? Refer
to https://wiki.ubuntu.com/KernelMainlineBuilds . Please test the latest
v4.0 kernel[0].

If this bug is fixed in the mainline kernel, please add the following
tag 'kernel-fixed-upstream'.

If the mainline kernel does not fix this bug, please add the tag:
'kernel-bug-exists-upstream'.

If you are unable to test the mainline kernel, for example it will not boot, 
please add the tag: 'kernel-unable-to-test-upstream'.
Once testing of the upstream kernel is complete, please mark this bug as 
"Confirmed".
