Yes there's a problem with the cable or the port here.

Looking at Dell's website they seem to confirm the built-in HDMI port on
the Optiplex 3000 only supports HDMI 1.4, so it can't do 4K60Hz. To get
HDMI 2.0 support on the Optiplex 3000 you need to be using the optional
add-on port which screws into the rear panel. If you didn't get that
optional second HDMI port with the machine then your only solution will
be to use the DisplayPort.

Please confirm that the problem is originating at least as low as the
kernel:

  grep . /sys/class/drm/*/modes

There is also evidence in XorgLog.txt that the deprecated 'intel' Xorg
driver was used in the past but your inxi seems to show the correct one
'modesetting' is in use now. Can you provide a full system log to
confirm?

Hi, thanks for the reply.  My existing cable claimed to be 4K HDMI 2.0,
but in the interest of troubleshooting, I ordered and received a new
HDMI Cable:

"iBirdie 8K HDMI 2.1 Cable 6 Feet 8K60hz 4K120hz 4K144hz HDCP 2.3 2.2
eARC ARC 48Gbps Ultra High Speed"

This has not resolved the issue.  I have successfully used 4K with other
OSes on this display. Here is the display spec:

I manually forced the HDMI input on the display to use HDMI 2.0 (It was
set to Auto). This required a reboot, and again, no luck setting a 4K

The most likely explanation here is that the kernel is not offering 4K
as a supported resolution, which you can check by running:

  grep . /sys/class/drm/*/modes

If it's not listed there then you need a higher quality HDMI 2.0 cable.
Or even better to use DisplayPort instead.

Status in linux package in Ubuntu:
  Incomplete

Bug description:
  This GPU should be capable of 4K resolution, but it's unavailable.

  Tried manually adding modeline with

  xrandr --newmode "3840x2160_60.00"  712.75  3840 4160 4576 5312  2160 2163 
2168 2237 -hsync +vsync
  xrandr --addmode HDMI-3 "3840x2160_60.00"

  Still fails to set 4K resolution.

  Upgraded to signed kernel 5.19.0-28-generic #29~22.04.1-Ubuntu, added
  linux-modules-extra, ensured that system is loading i915 kernel
  module.
