Looks pretty good. But a few more things to keep in mind:

1) There should be *some* body text describing the patch, even for a very
basic change which is explained in full in subject. This goes first
in the body, separated from the tags with a single empty line.

2) Signed-off-by and other tags go at the end of the body, right before
the "---" separator. Don't put anything between the SOB and the
separator. I realize that this was just a question that you wouldn't
make part of the submitted patch but still: It broke your example...

3) There is a place for questions about the patch which you do not want
to be part of the commit message: Right After the first "---"
separator. Anything yout put there will be ignored by git when the
patch is applied, just like the diffstat git includes by itself.

This is useful if you need to explain something to the maintainer
regarding how to apply the patch, or if ytou have questions about it
  which are irrelevant if applied.

So the end result would be:

Subject: [PATCH] INITEST: Documentation: Explains how INITEST works.

This is a test patch.

What exactly do you mean by "right format"? What do you want reviewed

Because almost always, exchange will corrupt patches.

Send one to yourself to try first.

But if you can use exchange as an imap server, great, use git send-email
that way and see what happens (again, test on yourself first.)
Instructions for how to set that up can be found in the man pages and by
asking your mail admin for the imap details.

Documentation/email-client.txt doesn't talk anything about the

One is a straight 4.5.0-rc7 (and why are you using that and not the
released 4.5.0, anyhow?), and the other is 4.5.0-rc7+ with a plus sign,
which means you've changed the source tree (what did you do to it?)

"4.5.0-rc7" is different han "4.0.5_rc7" :)

That's the issue right there, the module was built for "4.5.0-rc7+" and
you are running "4.5.0-rc7". Notice the "+" character? That's the
problem here...

Run the same kernel version as your module and all should be fine.

Note, the "+" means that there are commits past the last tag in your
tree (or that it has been modified in some way since that tag). So
there is a real difference here, which is why insmod is failing.

If you really think you can ignore this, you can always tell insmod to
ignore the kernel version check. Read the man page for all of the
details there.

good luck!

I do not understand these two sentences.

You said you tried to load a module built for the 4.0.5-rc7 kernel tree
(which is a very odd kernel version number, as I don't recall ever
making a -rc7 for 4.0.5, which was released way back in June of 2015,
but that's another issue...) In order to do that properly, you have to
be running that specific kernel version at the same time so this will
work properly.

So, what specific kernel version are you currently running? Does it
match up with the kernel version of the module you are wanting to load?
(exercise for exactly how to determine both of these is left for the
reader...)

What Greg is telling you is that for you to be able to insmod a module
you need to be running the same kernel version as the source code
against which you have compiled your module.

If you are running kernel 4.0.0, you won't be able to insmod your module
that has been compiled using the "4.0.5-rc7+" source code.

While inserting a kernel module, insmod is failing with following error.

insmod: error inserting 'page_walk.ko': Invalid module format.

I verified the modinfo of "page_walk.ko" it is showing version magic

Linear address: Also virtual address. This is Address CPU can address. This is
0 - 4GB under 32bit architecture.
Physical address: This are actual memory installed in your system. Linear
address have to be converted to

paging to physical address.

A program image generated by compiler & linker uses Virtual Address . During
execution of the program CPU + MMU
translates or maps each virtual address to physical address.

Net-net this is architecture dependent and would highly recommend reading
Intel’s Architecture Manual for your reference.
