Bug 18111: Swap framework_name and frameworkcode

With the frameworkcode changed to an empty string, which really
is closer to the physical reality, some javascript somewhere
breaks. This means the spinner does not appear when you confirm
that you wish to import it.

TEST PLAN
--
1) apply first patch
2) export default framework, doesn't matter format
3) import default framework using that file.
4) answer yes or whatever it is.
   -- spinner does not appear.
5) apply this patch
6) import default framework use the same file.
7) answer yes or whatever it is.
   -- spinner does appear.
8) decide if you wish to find and fix the javascript conditions.
   if yes -- do so, and then obsolete this once you have.
   if not -- sign this off after running koha qa test tools

In both 3.22.02.00 and 16.05.09.000 the "summary" search results do not show
item availability correctly, with ALL items showing as " items, x<>
available", even if the item is currently checked out.   Clicking on the item
title brings you to the details, where it shows correctly as checked out, and
the OPAC search results also show the item properly as checked out in both the
summary search results list and in the individual item details, it is only in
the Staff client summary search results that it is improperly displayed.

Bug 14353 - Show 'damaged' and other status on the 'place holds' page in staff

This patch adds a 'Status' column to the items table on 'Place hold' page.

To test:
- Apply patch
- In staff client, try to place an item level hold for items with Damaged,
  Lost, Withdrawn statuses.
- Verify a new column 'Status' indicating the item's status as appropriate.

Bug 13968 - Branch email hints are misleading

This patch replaces missleading notes with Reply-To and Return-Path fields
to reflect indicate falback values if they are empty (ReplyToDefault and
ReturnpathDefault system prefs).

To test:
- Apply patch
- In Home > Administration > Libraties and Groups, endit a library
- Verify that hints display for fields Reply-To and Return-Path, indicating
  their default values.

Hi Kyle,
I like this feature very much.

However, I get an Internal Server Error while systematically testing. It occurs
with following settings:

- Delete all rules in 'Circulation rules and fines'.
- Set a Default limit by patron category, e.g. Student 2 current checkouts , 2
on-site-chekouts

Checkout to a student until the limit is reached. 

Result: 
Internal server error
Plack error log says:
Template process failed: undef error - Can't call method "description" on an
undefined value at (...)/Koha/Template/Plugin/Categories.pm line 32.

Without patch, it works fine, the message is triggered.

BTW: I can always check out one item more than defined by the rules. E.g. I
allow 2 checkouts (by any rule), the message triggers with the 4th checkout. I
would expect it to trigger with the 3rd checkout. This happens with and without
patch.

Tested on master:
With Plack and memcached, start an upload with tools/upload. Fine!
Stop the memcache service. With Plack only, start a second upload. Also fine!

The Denied status normally is triggered by a wrong authentification. A possible
scenario could be using memcached for the session data. If you are logged in,
so have a session, and after that you enable or disable memcache, the session
cannot be found any longer. So should trigger an authentification error; this
is more or less what we expect.
Since the upload button starts an ajax call to upload-file and upload-file does
only use check_cookie_auth and not get_template_and_user, you will just see a
Failed status here. Clicking on another menu link should redirect you to the
login page.

Mirko: Can you check if you used memcache for session data?
