Smartcard Activkey doesn't work anymore. I use it without problem till version
qemu-kvm-1.0.1.

What this has to do with kernel?  The log clearly shows that qemu-system-x86_64
is unable to initialize (access to) the device in question, without providing
any hint about the reason (strerror(errno) anyone?), and you said it started
happening since version 1.0.1 of USERSPACE qemu-kvm.

Clear this is not a kernel problem. I thought this was the right place for
userspace tool bug.

I did some basic testing on nested virtualization on Intel x86-64 platform.
Will KVM support Xen as L1 guest in nested virtualization ?

When I tried Xen on KVM mode, I found VMX can't be initialized in L1 Xen
hypervisor.
I tried both -cpu host and -cpu qemu64,+vmx parameters in qemu-kvm command
line.
---some log in 'xl dmesg' command line in L1 Xen ---
(XEN) VMX: CPU0 has insufficent CPU-Based Exec Control (b299868c but requires
min 2299968c)
(XEN) VMX: CPU0 has insufficent VMExit Control (0200 but requires min
8200)
(XEN) VMX: failed to initialise.

In Xen source code, I found the following related to the mentioned error.

Avi confirmed there're two issues, and fixed 1st one (RDTSC) by the following
patch.

All processors that support VMX have that feature, and guests (Xen) depend on
it.  As we already implement it, advertize it to the guest.

Description:

Actually I have automated the provisioning and when it fails to start the vm it
removes the vm.img and vm.xml file. So there's no chance of trying a reboot
again.
Grepping logs is also not easy as the error is too intermittent.

I booted with iommu=pt kernel parameter and i stopped getting these messages.

I am experimenting with pcie kvm pass-through, so i ended using that parameter.
Sadly the whole host freezes when i power on the guest, but that is outside of
this bug-report.

 In my case, it only happens at POWER-ON, not when doing a RESET.
 So to reproduce it one must POWER-CYCLE.

I don't think that was actually not 100% accurate.

The addresses where the page-fault happens look strange: 0xf00420800.

This address is beyond TOM2 which means it does not point to system RAM. To me
this looks like this happens during firmware load (which explains why it
happens at power-on but not on reset).

I'm not sure that the vendor string is passed correctly.

Please verify that when starting the guest on an Intel host, /proc/cpuinfo
shows AuthenticAMD for vendor_id.

  qemu -cpu phenom,vendor=AuthenticAMD

Note that 32-on-64 applications will suffer a performance penalty when using
cross-vendor migrations.  You can mitigate this by using vdso32=0 on the guest
kernel command line (but this is slower than the default, when not doing
cross-vendor migration).

Trying the above options results in us being unable to start the guest on the
AMD host in order to test migration. It complains that guest cpu is not
compatible with host CPU.

The only method that works is by creating our own profile and adding it to
cpu-model definition on both the AMD and Intel hosts:

We recently added an AMD KVM host (AMD Opteron 6276) to our existing Intel
(E5630) only KVM host pool. We migrated multiple VMs (Linux CentOS 5.8 / 6.1 /
6.2) guests from the Intel hosts to the AMD host without issue. We rebooted
multiple VM guests (5.8 / 6.2) on the AMD host and migrated them back to the
Intel hosts. We then noticed that certain programs stopped working. We isolated
the issue to all 32bit binaries failing to run with Illegal Instruction
errors and we also receive trap invalid opcode on the VM guest console:

If we migrate the VM guest back to the AMD host we no longer encounter any
issues. If we reboot the failing VM guest(s) on the Intel host (after migrating
from the AMD host) the 32bit binaries work. We attempted to remove CPU features
that are presented to the KVM guests but still encounter the problem. Here is
the flags we have enabled on the guest:

flags   : fpu de tsc msr pae cx8 apic cmov clflush fxsr sse sse2 lm
unfair_spinlock hypervisor lahf_lm

This is reproducible every time. We attempted to use different OS installs
(CentOS 5.8, RedHat 6.2, SL 6.2) but the issue is present on all of them
(including their respective kernels). We also attempted to use different
models and we custom created our own model. Nothing made a difference.

 So, 1  0xE == 0 thus KVM assigns pci-bridge device to VM successfully.

Have you found this to be true in practice?  The test here is clearly wrong,
but immediately after this we test pci-sysfs resource permissions.  We only
test standard device resources, as we're not expecting a bridge to get through,
but I believe those should all be zero for a bridge, which will result in
denying assignment of the device because it has no resources.  If you have a
pci-bridge that actually makes it through that test, please include lspci -vvv

I have found bug in file virt/kvm/assigned-device.c

670 /* Don't allow bridges to be assigned */

This code doesn't check that device is PCI-Bridge. In my case

header_type is 1,  default value for PCI-Bridge
PCI_HEADER_TYPE is 14(0xE)
PCI_HEADER_TYPE_NORMAL is 0

So, 1  0xE == 0 thus KVM assigns pci-bridge device to VM successfully.

Bug detailed description:

Assign a VF or normal NIC to the guest, the guest cannot get IP address.
Note:1. assign the VF of  Intel 82576 NIC or 82599 NIC, the guest cannot get IP
 2. hot plug a VF to the guest, the guest cannot get IP
 3. assign a PF to the guest, the guest cannot get IP.

Reproduce steps:

1. ./pcistub.sh -h 08:10.0   ( to hide a device)

First of all, redhat version of qemu and kernel are outdated and heavily
patched - please address the questions about redhat versions to the redhat
support.  You can ask here or, more likely, in qemu related mailinglist or
bugzilla, if you can reproduce this bug with current upstream versions of
kernel and qemu[-kvm].

And second, for bugs in windows programs, please verify that this issue does
not exist on bare hardware, and ask their support channels.  It is not typical
to have this commercial development environment on an average linux developers
machine, so it is unlikely anyone will be able to help here.

I have the following problem reproduced on  centos6 and RH6. 

When I am doing a recursive grep on my Clearcase view using a 3rd party program
called Windows Grep. On every run, the VM would crash (windows kernel crash
where the cause is reported as unknown by Microsoft) up after the program ran
about an hour or so.

The qemu-kvm version is:

Hi, indeed, theoretically L3 should work; And also in practice - in my tests it
did work (albeit very very slowly). I'll need to look into this issue, and
check why this bug is happening now.

You should just be aware that even if it will work, L3 will be extremely slow,
likely to the point of not being useful. One of reasons is the lack of nested
EPT in the upstream version. An even bigger problem is the exponential
explosion of exits described in the Turtles paper, which is made much worse by
one particular part of the existing implementation: Right now, on every nested
entry from L1 to L2, L0 recreates all the fields of the vmcs (see
prepare_vmcs02()) doing a few dozen VMWRITEs. When L0 does it, it's fine - but
when we are nested yet deeper, and there is an entry from L2 into L3 and *L1*
needs to do prepare_vmcs02(), now L1 calls a lot of VMWRITEs and they all cause
exits, and all of this is extremely slow.

There are two things we could do in the future to solve this performance
problem - if people really need to use L3 (currently, even L2 isn't very
popular ;-)). One is to have some sort of exit-less VMREAD/VMWRITE, with or
without hardware support for this feature. The second thing is to emulate
VMWRITE differently, writing to vmcs02 immediately and doing far fewer VMWRITES
on nested entries (doing this correctly is harder than it might seem, as I can
explain in a separate thread).

Kernel oops backtrace

Trying to run KVM with a high level of nesting works fine for the first two
levels of virtual machines (L1 and L2) but leads to the attached oops when
attempting to boot into L3.

The host system has 24 Xeon X5675 cores running Ubuntu 12.04 with kernel
3.2.0-21-generic. The nested VMs are restricted to one of these core. They all
run resized clones of a Debian wheezy (testing) image with kernel
3.2.0-2-amd64.

In the Turtles paper, nested virtualisation has been presented to support Ln
and includes L3 in the diagrams but the evaluation only went up to L2. The
kernel documentation nested-vmx.txt doesn't list this as a restriction but
doesn't mention L3 or higher either. Hence, it is not clear if the
implementation is supposed to support L3 at all, at the moment or in the
foreseeable future.

I tested the same device here and got the same error as you reported.

Good to know, that Xen doesn't have any problem. This really means, that it is
most probably a kvm or qemu or AMD iommu bug.

I have same problems with pci passthrough, while my pci-e video card hd7750
with hda(01:00.0/1) and integrated network card(05:00.0) succsessfilly assigned
in guest, when i try to add usb controller, i get same error about FLR:

with xen it works fine.

Meanwhile I detected, that using the boot option iommu=pt (or additional
iommu=1) shows the same problem (regardless of suspend/resume). If the VM is
started after the first virsh start / virsh shutdown cycle with virsh start
again, the device is dead in the VM. Host must be rebooted to get it working
again.

If the options iommu=pt (or additional iommu=1) are not used at all, the VM can
be restarted without any problem coming up.

1. Host booted w/ iommu=pt iommu=1
2. virsh start
3. virsh shutdown
4. virsh start
5. - Passed through device in VM is dead.

Strange, looks like the patches did not take effect at all.  The opcode is 0f
6f 02, which should have been decoded as movq.

No guarantees of course, but this is just 3.3-rc5 with a few patches on top.
