I can not reproduce the problem if I create new file from scratch in both
version (3.6.5.2 and 4.0.1.2). But, if I try with test file attached above in
3.6.5.2 or 4.0.1.2, formatting does not look ok. Odd.

I don't know what exactly is the issue here, but since it works fine from
scratch in new version, this issue can be closed as fare am I concerned.

So if I understand it correctly: you can reproduce same behavior using
LibreOffice 3.6.5.2 (Comment 4), even when you create a new file from scratch.
But when you try using LibreOffice 4.0.1.2, you can't reproduce anymore from
scratch, (but still with the test file)?

Good question. I really don't know. I know a new feature in LibreOffice is
"Allow different header and footer on the first page without using a separate
page style." (http://www.libreoffice.org/download/4-0-new-features-and-fixes/)

Same problem in 4.0.1.2 also, but only editing the writer file attached above.
When I create new, similar file, it works fine. Has been something changed with
odt format ?

LibreOffice (3.6.5.2 or 4.0.0) and try to reproduce this behavior from scratch?

I can confirm this with the test document you attached; But I can't reproduce
it by creating it by myself.
