The "read-only mode" toolbar should be able to be turned off permanently.

Seeing the big yellow bar is likely to confuse users of forms who do not
understand that "read-only" is the required mode for entering data in forms.

I also find it annoying that it actually blocks off the top of my form, making
it impossible to enter data until the darned thing is removed.

Users should have the option of turning it off for all documents in the
Tools-Options menu and also per document.

Another alternative might be to show an option in the toolbar along the lines
of "Don't show this again".

Also the toolbar seems to have been deigned with the belief that all users of
all documents would always prefer to edit the document. The "Edit" button is
large while the X to close it is almost invisible in comparison.
