If I give the exact definition of the fancy-pagestyle
\fancyhead[LE]{\small\scshape\leftmark}

\fancyhead[RO]{\small\scshape\rightmark}

The index does not grasp the scshape and gives uppercase-letters. Can
anyone reproduce? I can live with empty headers here, though.

As you indicated, I have tried to give a new header with
\fancyhead[LE]{} \fancyhead[RO]{}
just before the index and now it works! Thank you for this hint. In all
other places, markboth served me well.

I also noticed that the title or caption of the index looks slightly
different than, say, the list of tables. But I can live with that.

Many thanks for your reply, Herbert.
Before I wrote
\renewcommand{\ps@plain}{\pagestyle{fancy}}
in the preamble, the first page of the index gave the page number on the
bottom middle, as opposed to all other chapter starts on the bottom right.
The index seems to grab some other page styles different to those in my
document.
The second page does not get the pagestyle fancy as I defined it in the
preamble. The section title in uppercase-letters (as opposed to
\scshape) doesn't appear elsewhere in my document. I am confused.

English environment is changed to textenglish

Please consider the two lyx files attached.
It is a MWE of a book that includes mainly Hebrew but also some English.

The wrong thing here is that the last line is converted in LyX (via the Code 
Preview Pane) as
\begin{english}
Some text in English. This should be left aligned.
\end{english}

but when exporting `main.lyx` to tex (via XeTeX), this English sentence is 
formatted as

\textenglish[variant=american]{Some text in English. This should be left 
aligned.}

which causes the line to be right aligned.

Why is it? How to keep the English line of text to be inside an English 
environment?<>

Witg fancyhdr you can set individually header/footer at any
position in your text. No need for using \markboth
