Only time I have seen these was from dropbox. Try exiting dropbox if it is the 
culprit there is a way to turn this off, but I would need to check the archive.

I have a couple system dialogs which I want to remove.  They say “unknown”, and 
there are no close buttons.  I thought that a reboot would help, but no.
Are system dialogs part of the Mac, or are they unique to voiceOver?
