Title:
  Licenses: Once you've selected a license, you can't go back to "None"

Bug description:
  While testing out Bug 1279526, I noticed that you can't change an
  uploaded file from a particular license to "No License". This may have
  been by design, with the idea that "No License" was a temporary thing
  until you pick a license. But with Bug 1279526 we're changing the
  wording of "No License" to make it clear that if you're not choosing a
  particular license, then the default copyright rules apply, so it
  becomes more necessary to be able to switch to that.

  To replicate:

  1. Go to "Administration -> Configure Site -> General Settings", and
  enable "License metadata"

  2. Go to "Content -> Files" and upload a file.

  3. Once the file is uploaded, click the pencil "edit" button to open
  its editing form. It should be at "No license" or the equivalent.
  Change it to "Creative Commons Attribution 4.0"

  4. Click "Save changes"

  5. Now click the pencil "edit" button again. Change the License back
  to "No license" or the equivalent.

  6. Again click "Save changes"

  7. Again click the pencil "edit" button.

  Expected result: You see "No license" or the equivalent as the license
  selection.

  Actual result: You see "Creative Commons Attribution 4.0" as the
  license selection.

When you choose the option "Other license (enter URL)" in your personal
settings, file upload or other places, there is supposed to be a second
field that comes up in which you can enter the URL for that new license.
This isn't happening in Mahara 15.10+ and probably got lost during the
Bootstrap conversion.

To test:

1. Go to "Administration -> Configure Site -> General Settings", and
enable "License metadata" and "Custom licenses".

2. Change your default license in your account settings
/account/index.php to "Other license (enter URL)".

Expected result: Another text field opens up where you can enter the URL
for this new license.

Actual result: Nothing happens.

Other places where this happens: Any artefact where you can choose a
license.

It still works in Mahara 15.04 prior to the move to Bootstrap.

Title:
  Allow for user auto-creation via SAML for multi-tenanted Mahara

Bug description:
  SAML authentication in a multi-tenanted Mahara installation can only
  be used if "Match username attribute to remote username" is turned on,
  cf.
  http://manual.mahara.org/en/1.5_STABLE/site_admin/institutions.html
  #saml-authentication for security reasons.

  The current code base does not allow for auto-creation of accounts AND
  a secure setting in a multi-tenanted Mahara.

  The main problem would be sorting out what the username should be in
  the multi-tenant situation as they have to be unique, but all the
  names are coming in from different systems that almost certainly don't
  use the same or globally unique conventions.

  In a multi-tenanted Mahara instance it should also be taken into
  account what usernames that are created on the fly by SAML should be
  like to be unique. Using the email address as identifier might not be
  a good thing as users switch between institutions and thus they'd have
  to remember an old email address for internal login or even with SSO
  always have the old address show up in the user search.

  Another issue is that esp. in a multi-tenanted Mahara users might
  switch between institutions and thus should be able to keep their
  accounts. If accounts are always auto-created by SSO this might become
  less likely unless the "Auto-link accounts" option is turned on.
