Tested back and I found that the pagination is not correct for newly
added feedback.

For every feedback added it is taken as 2 feedbacks and it shows wrong
number for the feedback.

  Mahara 16.10dev

  When feedback is placed on an annotation block, it is called
  "Annotation", but it should be feedback.

  In order to make it look more like the "Comments" on an artefact, it
  should be changed like in the screenshot to include the "Comments"
  icon and then "Feedback" with the count at the end.

  The icon before "Place feedback" should be the one we use for "Add
  comment", but no text change.

  Currently, when you add an assessment but no feedback, an empty
  "feedback" item is displayed. Rather, the assessment should be
  mentioned in it. That would also be beneficial to have a log of who
  changed the assessment. Please see the screenshot for what it should
  look like (the icon in color though, couldn't get that to work in
  Firebug).

  We don't use the class "metadata" much anymore I think. It would be
  best to use text-midtone and give it the size we have for the
  "Updated" date, e.g. under "Latest changes I can view" on the
  dashboard.

  I am using Mahara1604 and see a bug at Users profile page Wall
  posting. All artefacts organized at an user's profile page is
  accessible by all other registered users without any permission issue.
  But when profile owner attach a file/image in his own wall post, then
  other users not able to view that image which explains access denied
  error.

  I verified this on Mahara 16.10. Another user viewing the wall post
  does not see the image, but just the alt text.

Tested it on FF browser and here are my findings:

Feedback and place feedback buttons are now shown on the page,  but when
we give few feedbacks the number of feedbacks are wrong and also when a
feedback is deleted it is still showing up and doesn't get deleted .

Additiona testing shows that lines 111-113 in
htdocs/module/framework/lib.php are problematic. When I comment out
