Yes - it needs new bearings in the pivot arm.

Beware that there are different versions of that tension pivot assembly, be
sure to get one that fits your car.  Also order the plastic cover that
protects the external face of the pivot bearing from road dirt and grime.

Replace it.  The bearing in the pivot is shot, and it will both knock and eat 
belts pretty soon.
