Yeah. You know, I think next time I place an order with Rusty I'll order a
spare wiper blade - not just the refill. And I'll carry it in the car. I'm
really thinking of the trouble a wiper going out could cause.
This reminds me of how with my dearly departed Saab 84 900 I actually
superglued the old wiper rubber back to the plastic spine. It was peeling
off from age. I was pretty low on money. It actually worked - I thought.
What I didn't see coming is that the rubber was so fatigued that it flopped
over on its side during each stroke of the wiper. This allowed the metal
grabber clamps on the wiper arm to scratch the windshield. DOH.

last set of refills I got---they're SWF brand, not Bosch, and the
instructions don't mention any plastic pieces.  They basically consist
of "remove old refill, remove metal splines from rubber, transfer
metal splines to new rubber strip, reinsert in blade."  The trick
comes in understanding why it's so important to get each spline on the
correct side of the rubber refill, and in the correct orientation to
the windshield, the first time around.

So I don't know why the Bosch package is so different and the
instructions so useless, unless the 201 and 124 pantographic
monowipers are different (which I don't think they are, although I
wouldn't bet my firstborn on it).

And the illustrations were near usless.

At least I didn't cut my fingers. That was one of the most common cuts I
would get as a young man working at a service station. You wouldn't know it
until you finished the work.

You should check the blade assembly each time you want to replace the  
rubber, too -- as the pivots wear (and they do, rather badly, even on  
Bosch quality blades), the blade will start to wobble and thump  
loudly as it moves in and out.  I was about to replace the  
transmission on my mother's when I instead bought a whole blade  
rather than just an insert.  Instant cure, works much better now.

Do watch though, there are two different length main arms on the W124  
at least (don't know about the W201), and if you get the wrong blade,  
it will hit the trim around the window, rough on the wiper  
transmission and your ears.

I do this on all my cars... its so much easier to install!!  The W116 is 
an exception... since it costs some crazy amount of money (think it was 
over $70) because they are stainless steel.

Brian, I feel your pain.  It took me about as long the first time.
Now that you know the trick it will go a lot faster the next time,
though.

Re what kind to buy, I have had bad results with Bosch blades from a
local store.  I see that Rusty's site lists the blades and refills as
made by Bosch, but when I ordered some a few months back, the ones I
got aren't Bosch but some other OEM (I don't have the package in front
of me).  So far I've been very pleased with their performance, and
they had good instructions on the back of the package as well.

I share your concern about the possible safety hazard if the refill
isn't secure.  Two hints: use a flatblade screwdriver to open up the
jaws of the blade just a hair before sliding the new refill in, and,
to make sure it doesn't slide back out, crimp the inside set of jaws
(closest to the windshield) around the refill with needlenose pliers.
(Actually, I use duckbill pliers [like needlenose but with flat jaw
faces] to prevent marring, but those are hard to find.)

Of course I bought the whole wiper not just the blade but the ease of 
installation seems to outweigh the slightly higher cost...

I never was able to find decent wipers (HArdware was different and "Improved).  
I tossed several on my car.

Than I was stuck by a revolation and bought OEM from Rusty, problem solved!!
