If it listens on a port, use check_udp to listen on port 4500. I don't understand your second question. You can create a VPN tunnel using two routers or VPN apps like openvpn or freeswan running unix/linux. Alternatively, you can use DNAT and send all port 80 requests to your nagios server. This was not a question. You can create also a VPN between a PC running Linux and a VPN gateway with IPSEC. If you have a VPN you can do a ping to an IP address behind the VPN gateway to test if the VPN is up. For example you can use the LAN IP address of the VPN gateway.
