The insidesection/aftersection options in \setuphead works well. I have 
been used the following way:



\starttext

\chapter{title of the chapter}

\startcolumns

\section{Introduction}

\dorecurse{5}{\input davis\par}

\section{Methods}

\dorecurse{5}{\input davis\par}

%

\stopcolumns

\stoptext

I have read that the \start...\stop way of the commands are better for 
exporting  to another format like xml.

That's yet another reason to avoiding fixing the values of the 'm' and
'n' sizes. As of now, you have to first decide your font size, lookup
the 'x' and 'xx' sizes in font-pre.mkiv and do a little math... every
time you switch fonts. Instead I've come up with a function that
doesn't depend on hardcoded globals:

However I worry that measuring individual letter sizes to deduce the
actual font size may not be foolproof, especially with optical sizes
and so on. There must be some other method (at least via luatex) to
obtain the 'x' and 'xx' sizes as workable dimensions.

[NTG-context] regression in fontloader?

The following example gave with the generic fontloader from
2018-12-19 and luatex 1.07 two identical lines. With a fontloader
from 2019-01-07 and luatex 1.09 the first line is wrong at the left
side, the "j" is squashed into the next glyph:

This works only when you use a fixed width for the item symbol.

The textdistance keys controls the horizontal space between inline items
