None in between. I prefer 4ft front and back, but I'm pretty sure that
exceeds the regulatory minimums. I like to have a little room when
operating on equipment.

Side to side isn't that important.  Side to wall is, in that you need to
make sure you can open your rack doors wide and also give yourself room
for two people to mount equipment (one on each side).  Make sure you
have room in front and back of the racks that you can easily install and
remove your equipment.  Try to have adequate lighting in front and
behind so you can see what you're doing.

We have six racks of equipment. no spaces between the racks. It will mess up
your airflow

2. How far out from the walls to you have them (both for air flow and for
rear access)?

 

3 feet in the back, 4 feet in the front.

Side-by-side isn't an issue. Generally, in the USA, you are required to have a 
minimum of 36" clearance front and back.

My racks are set side by side touching each other (3 racks + stand up APC UPS). 
I've got room to open the backs but the wall is probably 6" behind the open

I can't say much about my current situation as we only have one rack.
However, at a previous position, we had a *bunch* of racks literally
side-by-side jammed so close together you couldn't fit a sheet of paper
between them. Of course, those racks had open bottoms with forced-air
cooling coming up from the raised floor (about 6" or so off "ground" level,
presumably for cabling and ventilation.) Granted these were running older
servers (old Compaq Proliants - quad PII Xeons, IIRC in some of them) so the
cooling might not have been as critical as it is with newer CPUs. That being
said, it was almost always cold enough to hang meat in there (OK, not
really.. more like about 60-65 degrees F.)

My placement is simple assuming that environmental (power/HVAC) and network
cabling is adequate; I make sure I can open the rack doors completely if
installed, I make sure I can slide the servers all the way out for
maintenance (don't want to have to move a rack to install a server), and I
make sure I don't block doors/vents. Side to side I butt them right up
against each other. Unless you have a specific need to get into the rack
from the side, I've never found it to be necessary to spread them out that
way...

1) None.
2) Check OSHA and any local building or safety regulations.  Those will
drive the minimum distance.  That minimum will exceed what you need for air
flow.  Keep in mind those regulations will usually apply from a door open
perspective. (start the measurement from an open door, not closed)
