I wanted to make clear to everyone that while I am certainly no expert, I am interested in PACS servers and RIS systems only here. I know that there are many good open source DICOM viewers. One important consideration for medical imaging in the US context is FDA approval. While you can get away without it for "R&D" if all work is done at cost... if you are on the service provision side of the equation and you do more than a hand full of implementations you will need to work with something that has been approved. Most imaging falls under the first level of approval and is not too onerous to achieve.if you want to handle mammography then the stakes are much higher. Between DCM4CEE and Osirix (and Mitra, of course), most features of a PACS are covered (extremely well, BTW). Community support is also excellent in both cases). DCM4CHEE has some limited web access and a project in process to improve it, although I'm in the process of writing PHP access for my hospital which will be GPL when it is mature enough. Osirix is limited to Mac platform, There doesn't seem to be many installations of a significant size, though, although - I may be able to offer ourselves (550 beds) as a reference site at some point but need to tread gingerly. Of course PACS is of little benefit without RIS, & I'm not aware of any OSS project that specifically covers RIS outside of whatever is included in Vista.
