Team,

Several people have told me that they got an error when they tried to use
the form.  The problem is that each form has the same title, so the wiki
considers them identical and generates an error.  I updated the title with
"fields," in angle brackets, for the name of the project and the milestones
affected.  This should produce unique titles, even if the same project
submits multiple requests.

Team,

After processing our first two exception requests, I found that using email
was awkward and messy.  It's also hard to keep track of exception requests,
since they're buried in email threads.

Therefore, I created a wiki template and enabled it via a button that can
be used to create a new form, based on the template.  Each form that is
completed will be listed in a table.  This is identical to how the release
plan summary is implemented.
