That might be why you never saw it before. Try to create the same failure on a test system and see if the cancel causes a crash. I had the same occurance on an 8.0.5 database. We had a long job running , also a trucate, that was killed. When this happened the database core dumped and died. We looked it up on Metalink and found that it was a known error with that version of the database. It appears to happen with heavy IO intensive operations. Its too bad you have the same error on 8.1.6.
