Does anyone know where I can find a good explanation of transition during childbirth? What do you usually tell women about transition in childbirth education classes (or when asked)? I probably can't help you much here, but I had to share! Rhea Dempsey (www.birthingwisdom.com.au) has a trademark 'catchphrase' that sums it up all so well and I know many other midwives love it too. It's not specific to transition and can happen anytime in birth (but usually in transition!). She calls it a 'crisis of confidence'. Isn't it fantastic! Talpa, the new television station launched earlier this month by the billionaire creator of Big Brother John de Mol, will broadcast a new series from Sunday in which a contestant is due to give birth six weeks into the show. A spokeswoman for the social affairs and labor ministry confirmed a report in De Telegraaf daily on Saturday that inspectors were examining a request by Big Brother producers for the newborn baby to be allowed to appear on the program. The Netherlands has strict rules governing young children acting on television, in films or on the stage. The ruling Christian Democrats have condemned the idea of a birth on the live show, but the 27-year-old pregnant contestant identified only as Tanja defended the idea. The show's director Hummie van der Tonnekreek said Tanja would be well looked after in the Big Brother house, where a group of 12 strangers are locked in together and gradually voted out by the audience. Could everyone who is doing something for the 2005 National Caesarean Awareness Day please register you event with CARES SA. We are long term planning for some serious funding in the future and need to keep track of just what is happening and where...what are you doing, who have you organized to speak, what focus is the day and most importantly are you adhering to the aims and philosophy set down in 2001. It is important that even if you are getting together for an informal chat with some cs/vbac mums to register. If we can show that there is continued even growing interest in this event then we can achieve funding for the a organized national event in the future. Hi Emily, I don't think there is much evidence to support not doing VE's in labour at the moment. But I guess as most midwives will tell you that with experience you can learn to judge where the woman is at without invasive VE's all the time (of course there are always a few surprises!!) I find that the pigmentation which travels up their bottom crease as they dilate is a good indicator, as well as listening to their sounds, the things they say and their behaviour. If they were pushing and there were no signs of descent after a time then I probably would do a VE. I think the worst part of doing one to confirm that a woman is fully (apart from the discomfort to the woman) is that then the whole time factors comes into play. eg. in some hospitals they only "allow" 45 mins for a primip to push and 30 mins for a multi. So by avoiding VE's or putting them off you can buy some time for the woman. Hi, I've just returned from a clinical placement in SA where I spent a mindblowing three hours in an incontinence clinic in an outpatients unit at a major hospital. The mindblowing element was the following statistics (copied from one of the handouts): 39-49% women tear or have an episiotomy needing sutures 0.5 - 2.5% have a 3rd or 4th degree tear after vaginal childbirth that is visible 25-35% after first vaginal delivery have a concealed or closed 3rd degree tear, not visible Listed as contributing factors were: 1st vaginal birth forceps/instrumental delivery long second stage 1 hour big baby 4kgs tissue type, short perineum, epidural, uncontrolled pushing, rapid delivery, midline tear or episiotomy The nurse practitioner stated this was all evidence-based information and recommended c/sections to women who had had previous 3rd degree repairs - these were the ones who knew about their tears obviously. The handouts do not give references and as yet I have not had time to begin researching. Are you all as mindblown as I am?? What do you think - are 1/4 - 1/3 of us walking around with damaged anal sphincters and not aware of it?? Where does this sort of information lead us - if our bodies are so inept at giving birth then all first babies and subsequently all babies should be born by c/section. Sue

Hi Sue, Take a big,deep breath, and anothernow.think: who stands to gain most from this data who stands to lose most ?? You are mind-blown because in yourheart, you know better ! Just to recap... Based on what you learned inone3 hour session, with no reference material to back up the information, you are thinking thatthe only wayto prevent this catastrophe would be to perform vaginal bypass surgery on all women surgically remove all babies from their mothers. Thisis how Obstetricians are brainwashed intorescuing mothers babies from damagingone anothers bodies during the perfectly normal, natural birth process. Perhaps checking the stats, references research before you lose confidence inwomens bodies ability to accomplish (without horrific damage) what we are born to do mightrestore your faith! I'm sure there are a million variables involved, could their birth practices, labour management techniques, promotion or lack of active birth principleshave any bearing on their stats ???
