Sorry - forgot to email this morning. I managed to solve the issue - someone 
turned on the "require content approval for submitted items" setting in the 
form library settings, which I discovered purely by accident :)

We have an infopath form published to SharePoint that the business uses, with a 
workflow that has a number of approval steps.
